I attended the store on June 8, 2021, and spoke with Amit. I requested an estimate for my insurer respecting a CPAC and accessories. I provided my Physician's documents and was told he would forward my estimate. I did not sign anything at this time and emailed a photo of the serial number of my present machine as requested. I received no response and after 6 weeks I returned to the store speaking to Amit. Inquired re an estimate and he advised that he could complete it in 1-15 minutes. However once he realized I was under the ADP Ministry of Health (MOH) funding, he indicated that he could not supply an estimate until he knew how much was covered by MOH. Note I later found that the funding was a pre-set amount, On October 4, 2021, Raphael from Capital called and left a voice message to call the store re my order. I returned the call the following day and was only able to leave a voice message. No response was received and I called again on October 11, 2021, and canceled my order advising that I would seek a new provider with a higher regard for customer service, I initiated contact with ******** by ******** ***** and met with the Agent on October 28, 2021, at which time I provided the Physician's documents and was emailed a quotation later that day. My insurer approved what they would pay for and I attended ******** in mid-November to initiate the purchase and the application was sent by them to MOH - ADP. On January 4, 2022, I received a letter from MOH denying my ADP funding application. I contacted ADP Section and was advised that Capital Home had submitted my application on September 16, 2021, and after completing some corrections, my funding was automatically advanced to them on September25, 2021. Two days later, I contacted the ADP Investigative Section to report the failure of Capital Home to contact me and return my funding to MOH for goods not supplied. Confirmation was received on February 24, 2022 that my funding had been recovered and applied.
