      On March 8 2022 I paid Revita Medical $2095 for initial payment for ***** **** treatment for ******** *********** at there Waterloo clinic. I entered a contract with them for a treatment program that consisted of weekly treatments for 6 weeks followed by a rest period of 4 weeks with a cycle of a minimum of 3 treatment periods for a total of 18 treatments as a minimum with additional treatments during the calendar year. I received the first treatment cycle and on July 5 2022 paid an additional $1500 for the remaining treatments. I received 1 treatment and then future booked appointments were cancelled by Revita citing staffing issues. I traveled to Oakville for 2 more treatments but the distance I had to travel there was impractical. I was informed the Waterloo clinic was closed and they were in the process of moving to a new clinic in Waterloo. After repeated calls to have my treatment resumed with no indication from Revita of when a clinic would be open I requested a refund of the payment I made for service not rendered. In my repeated calls requesting a refund I was always told I had to talk to their financial manager Vilma G. On the very odd time she did reply she always told me the Waterloo clinic was reopening. I was never offered a refund.

      I visited Revita Medical on 7 November 2022 to seek treatment *** ******** ***********. The nurse in attendance called Karl told me treatment was not covered by ****. He suggested I could make payment by debit or credit card. I said I would pay either way; then he said he wanted to see my credit card to make sure it was current. He checked it and returned it to me after noting the details. Next, I was made to sign 3 sets of what the nurse called “treatment forms”. I ********* signed without having analyzed/read them because he said he would give me copies of the signed treatment forms for me to read in detail later as my slotted appointment time was running short. I was told I would get copies on November 11 at the next clinic appointment. He also advised me to check my email. When I checked my email much later I found an email from ********t stating that my loan application via Revita to cover 12 months treatment had been approved and that monthly payments of $375 would be made via my credit starting from December 2022 till November 2023. I responded to the email telling ********t that I had not applied for a loan and that they should cancel it. I also advised them that the bank had cancelled my credit card. I realized something was amiss. ********t has told me the loan can only be cancelled via Revita. I have phoned and written to Revita asking them to cancel the loan as I am unable to carry on with future treatments due to inability to pay. I have also advised Revita that I will pay for the November 2022 treatment costs, their overheads and the loan cancellation fee(s) despite the fact that I did not apply for the loan. In short I am asking for billing adjustment to reflect only total cost for services rendered to date; I will not take any further treatment ** * ****** ******. They have not responded despite the fact that receipt of the registered letter was confirmed. Please help to get an amicable solution.

      in March i paid Revita $3500 for ED treatments to be given over the course of a year. this was financed from their affiliate *********. After only 5 treatments they closed their Ottawa office and cancelled future appointments. August 30th- i emailed Revita to inform them i was removing authoization for ********* deductions from my **** Sept 28th- called Revita spoke with Finance Manager. She stated she would start process to have auto payments stopped Oct 31st-opened dispute cases with ***** Bank Nov 8th-noticed auto payments to ********* were still occuring . Called Revita but Finance Mgr would not return my call Nov 18th- attempted to contact Revita again but no reply so i called **** and deactivated my card. ********* contiues to send me requests for reimbursement for treatments not received. Revita has to either send ********* money back or reimburse me so i can pay them back.

      Start date January 2022 Revita Medical was to provide medical services at a cost of $3500. This treatment was to take place weekly or a period of 12 mths. Half way through the treatments the office closed and has never reopened. I spoke with several people who assured me that the office would be reopening Finally for the last 3 months they have informed me that they will be refunding me my fees but I have not heard back from their Finance Manager in this regard. In the mean time they continue to charge me month fees from my account. They are using ********* who informed, me if I try to stop payment ,they will file against my credit bureau.       Scheduled a visit on April 19th for evaluation ** ******* **********, it went straight to payment and shockwave treatment. Payment history: April 19th $1,000 Apr 26th $750 May 4th $750 May 12th $ 1,000 Total: $3,500 CAD. Schedule consisted on 18 sessions separated in 3 groups of 6 sessions each with a 2 months rest between each. 1. 6 sessions - 2 months break - Completed 2. 6 sessions - 2 months break - To start in august 3. 6 sessions - 2 months break - To start in december Last session on May 24th with a 2 months break to retake in august. I have been calling the business to reschedule and they have mention Vancouver clinic is closed with no opening date. Was supposed to receive a call from the Finance Manager Vilma G. and after a missing call there have been no more attempts. Email information request has been sent since september 30th with no success. At this point, I have only received 1/3 of service paid in full and no feedback from the business.
