My health insurance clearly states:? There are no PPO network requirements.? All licensed health care providers are accepted.? Physician services reimbursement: Up to 125% of ******** Allowable ? Medical Facilities Reimbursement: Up to 150% of ******** Allowable Yet, the office of Daniel Spriggs of The Perdido Bay Medical Group refused to file my insurance claim for a doctors appointment on 1.24.23. Instead, I was charged $150 for an appointment to see a doctor, who has previously been reprimanded and fined by the ************* of Medical Examiners.I asked for the following information in order to file the health insurance claim myself, but my request was refused/ignored by Daniel Spriggs:An itemized bill from the provider/facility, which must contain:1.Patients name 2.Providers name and tax ID#***********, and CPT codes for services provided 4.The date of service 5.Charge amount for each service provided So far Daniel Spriggs has not provided me with the information needed, so I am requesting a full refund of the $150 paid for a substandard doctors visit on 1.24.23. I wonder why a doctor would refuse to provide such basic information for insurance billing, but maybe his medical license is in the process of disciplinary action again?
