I am starting the complaint on behalf of my wife, ******************************* who is out of town on a family emergency. She has not been a patient at Cottonwood Smiles for approx. two years. In Feb 2023 we received a "Past Due" bill for services for $211.00. ******* contacted the office and was assured that the bill was not only not past due, but an error. Since then we have received 2 more notices - now threatening collections.We have repeatedly attempted to contact the business with the goal of getting an itemized bill so that we know what we would be paying for. Each time we reach an answering service - their response is that they will have someone call. Nobody ever calls back. I have repeatedly visited the business during stated work hours only to find the business locked - no-one there.Recently I have called and left messages daily with no response. I have also emailed Cottonwood Smiles via their website with no response. We have notes documenting call times. The answering service also assures me that all calls are logged.
