On Friday - April 22nd, 2022 I scheduled a COVID 19 testing with PremiumHealth (via their website) to be performed on May 10th, 2022 at one of their testing sites located at ********************************************************************************** and they charged me $89.00 right away. Due to unforeseen circumstance I cancelled the appointment an hour later on the same day. I got the message on the same day that the booking has been cancelled (see attachment) - I have contacted the business and asked them for the refund, they told me to fill out the form and I did (On April 25th, 2022: see attachment) and I have been informed that it will take 7 - 10 days to get my refund. But I haven't received my refund. Again I have reached out to the company and talked to a lady: ***** on May 10th, 2022 and she told me to fill out another identical form like the first one and I did. And as of today, May 31st, 2022 - I haven't received my $89.00. Hope you can help me! Thanks in advance - Warm regards: Nha - P.S: This is the reference#***** of my booking.
