On July 7, 2022, I saw The ***************** for a crown at San Tan Valley Smiles, a business that according to my ************************ is an in-network provider. Before I went to see the dentist I looked up my ************************pay with Humana, PER HUMANA WEBSITE:Crowns Crown Porcelain/Ceramic Copay $430 Major Restoration (shrd) Freq In Net (per 5 Years) 1 One per tooth per every five years Does not apply to deductible However when I checked out I was told that this was not covered by my plan, I explained that it is covered and what my copay was, the person then told me that when the insurance company paid my money would be refunded. I have been going to this. Clinic for 12+ years and have never had an issue, so I paid the $972.00 up front, knowing that the difference above my copay would be refunded. When I received the explanation of benefits from the insurance company, it states that the Dentist *********************** is out of network and the claim was denied. I have contacted the ****************** several times and asked them to re-submit claim under the correct dentist and was told the claim was submitted a 2nd time on July 20th, 2022. I have checked my Humana website and it shows they have not submitted corrected claim. I have continued to see this dental clinic because they accept my dental insurance. Why was this claim billed under a Dentist that I did not see? If only certain Dentists are considered in-network, this should have been communicated to me, and I would have scheduled my visit with an approved provider. The whole point of dental insurance is to help pay for dental services. My plan specifically states that my co-pay for the crown is $430.00. I would like the difference that I paid to be refunded, as I was told it would be.Initial Complaint

I was charged for an entire service that was not rendered within the span of 5 months. I required braces and dental implants over the span of 18 months but lost my job, sold my home, and had to move to a family members home for survival and did not complete the services. They didnt send me any notifications through the mail and sent my **** to a collections agency for the entire amount for which I did not receive. I got braces but never returned because I didnt have the money for those services and when I did return when I could I was told the entire **** went to collections. My teeth still have braces on them and I have not had the implants as the contrac states but was charged for the entire amount and not notified that it went into collections. I left a photo of my teeth with braces on them as proof.
