On 11-23-21 I signed a Treatment plan for $15,000. According to the attached account history, dated 12/19/23 I have paid $14,725. On the bottom, shows balance of $3445!!! No way!! I requested a copy of the treatment plan. What I received was actually 2 of them. The second one shows my signature for $4900. Apparently, they forgot something and decided it was a good idea to copy and paste my name to it. I was never told anything about any additional charges. This is a Dental Practice that I have gone to for many years, so I tusted them. They had 2 years to tell me, but opted not to. Now, I am questioning if the insurance payments were actually deducted correctly and actual charges are correct. I had asked so many times for a statement from *****, and when I did receive one, was told to disegard the balance because it was incorrect. Once, I realized that they copied and pasted my name, I will not contact them and let on that I know. I have spoken to several other dental professionals and this is not common practice. If the was an error on the original, I should I been told about it immediately and been given the option whether or not to proceed. Please contact me if any other information is needed.
