Over 2 years ago I completed counselling for a DUI. On November 3, 2022, the fee for the revocation package was paid to Perceptions Counseling. In the company of the administrative assistant, and a receptionist, the owner, *** ********, assured me that they had all the information needed to submit the package and I would be contacted by the MVD in 45-60 days. When I did not receive communication from the DMV, well past 60 days from submission, I called MVD and was told that only the substance abuse evaluation had been submitted, they had not received the alcohol evaluation or proof of completion. I then contacted Perceptions Counselling and was told that the oversight would be corrected. After 2 weeks, I contacted the MVD to check the status, nothing had been submitted. I have since been told that more information was needed, to which I resubmitted information that had been provided...over two years ago. The administrative assistant has reassured me that all information is in place and only the owner can submit/send the email. I have a certificate showing completion of alcohol counselling, which I could not have completed if an evaluation was not done to begin with. The last conversation I had with *** ********, I was yelled at, threatened, (she said she would call a "********" and the MVD to tell them she did not like my attitude...which was interpreted as to be, 'If you don't put up with my abuse, I will not submit the necessary information for you to get your license back.') She hung up on me when I questioned her actions...and my HIPPA rights were violated...she asked for and I shared personal information while someone listened, without my knowledge. The owners' less than professional actions were so unbelievable, I put the phone on speaker so that my mother and aunt could attest to the owners' outlandish actions. *** ******** has left me traumatized and without a license. I need the alcohol evaluation and proof of completion to be submitted to the MVD as soon as possible.
