On 6/25/22 my wife ******** ***** was admitted to the hospital for hip surgery, she was there for a week. Approximately 2.5 weeks ago we tried to get a UB04 form for our supplemental insurance, they told us that our insurance agent would have to call and request that form. We called our agent from State Farm and informed her, ( **** ****). She called to request it and met with nothing but rudeness and attitude, ( someone called *****, ********?). Our agent kept trying and was directed to a ******, unfortunately we have met with nothing but problems here as well, she would not respond to my emails. As of July 29, 2022 we still have nothing to show for our troubles. This is a form that *** is entitled to as it is part of her medical records, we have not had any problems getting this form from other medical centers, just this one. It’s not a top secret document. They have our agents contact info as she has sent it repeatedly. Thank youBusiness response

Mr. ***** was provided with a copy of an itemized statement on 7/22/2022. Mr. ***** insisted on obtaining a UB04 form and he was informed of the proper process at that time.Kern Medical's Business Office contacted the patient daughter who is in fact the patient's agent to explain in detail that our office does not provide UB04 forms to our patients. The patient's agent was informed she needed to complete a release of information form with the patient's signature and request a copy of the patient's medical record (admission/discharge dates and reason for confinement/diagnosis) to obtain the information she is requesting in lieu of the UB04 form.We apologized to the patient's agent for all of the trouble they have experienced and offered assistance with any further concerns. The patient's agent was very appreciative for us reaching out and resolving the patient's needs.Please let me know if you have any further questions.Thank you,**** *****
