      Good Afternoon, 

      Customer D. ******* was contacted via telephone at phone number provided on complaint. I attempted to obtain further identifying information about her account in order to have her request for refund for services reviewed by ARMC administration. Mrs. D. ******* advised she did not want to provide further identifying information. Per Mrs. D. ******* she wants a refund for the damage done to her jacket. I explained to her the process of obtaining reimbursement  for damaged items would need to go through San Bernardino County Risk Management department as a claim. She voiced understanding. Per her request I emailed her a claim against county form. Customer had no further concerns at the time the call ended. 

      Please let me know if any further information is needed.

       I am rejecting this response because:

      I have yet to receive a form from arrowhead for them to pay for damages to my property.

      Good Afternoon, 

      Attached to response is  the claim against county form that was sent to D. ******* on 1/10/23 at 10:00am. During phone call with patient she requested the form be emailed to her instead of sent through mail.  A second email was sent to patient today 1/13/2023.

      I dispute the validity of the alleged debts in its entirety reporting on my credit reports. Further I request verification of the alleged debt, including, the name and address of the original creditor, along with a copy of a signed contract with the creditor. Where’s my HIPPA release form? I am aware that you have directly violated the regulations of the HIPAA Act of 1996. I never signed a HIPAA form to authorize your company to have access to my medical records or procedures that I went through. Additionally any attempts to harm my credit history and rating by updating or changing dates after you have been informed that the debt is expired, is a direct violation of the FDCPA. Any abuse to my credit rating on your part will be met with all recourse available to me by the law. You are illegally attempting to collect an alleged debt and money that I do not owe. As per the law, cease and desist all collection activity, including credit reporting.

      On 11/25/2021, I sliced the top right of my thumb with a mandolin. I went to the nearest emergency room which unfortunately was Arrowhead in Colton/Rialto as the bleeding did not stop. The nurse was not able to stop the bleeding after attempts to stop it by applying pressure. No other measures were taken and so the nurse dry wrapped my wound and sent me home. On 11/27/2021, I went to a doctor closer to my home in Foothill Ranch and he advised me to never step foot inside Arrowhead emergency room ever again. He advised Arrowhead did not properly take care of my wound. It took over an hour soaked in hydrogen peroxide just to take the bandages off because the blood was rock solid stuck on the dry bandages that Arrowhead emergency room wrapped. Since my health was not properly taken care of, I wish to not get charged for any bills. My wound is going to take months to heal because of the way my wound was wrapped by the people inside the Arrowhead emergency room. Please advise as to how the policies will change to better care for people. Thank you :) -****
