The gym membership checks for July 25, 2019, August 8, 2019, February 2, 2020, February 27, 2020, March 5, 202, April 9, 2020, May 7, 2020, June 11, 2020, July 16, 2020, August 13, 2020 are invalid and I would like to request a reissuance. No checks were received for September 2020, October 2020, November 2020, December 2020, January 2021, February 2021, June 2021, and July 2021. There was also a missing check *********************************** for over ****. Please reissue the checks as soon as possible.Business response

As of receipt of this complaint, we have been in contact with the Member and Personal Representative. Thank you Resolution Summary The following checks mentioned in the complaint have been reissued to the Member. February 27, 2020; March 5, 2020; April 9, 2020; May 7, 2020; June 11, 2020; July 16, 2020; August 13, 2020; September 2020, October 2020, November 2020, December 2020, January 2021, February 2021. As of 9/17/21, we are in communication with the Member and Personal Representative in connection with confirming checks cashed for the following months listed in the case complaint. July 25, 2019, August 8, 2019, February 2, 2020 As of 9/17/21 records, we have not received the reimbursement request for listed months. June 2021, and July 2021.
