      I had been a patient of ****************** since 2017. I first came there due to a molar tooth that had fallen out no pain just fell out. I came to her because there was a bit of it still attached to the gum. I did not pull it out to prevent injury to myself. I went to her and said to have it removed but she insisted to save the dead tooth and had it root canal. ***** then Months went by and the gun had a recurring swelling I believed it was a recurring infection and went to her aid. She blamed me for not flossing the root canal enough, even though it was root canal and dead. Since then, often the gum has a recurring inflammation and it pains me. I called her multiple times about it but she never got back to me. I called and finally she answered and said it might be the wisdom tooth that was causing it. I did not believe her because she refused to have all my wisdom tooth removed during an annual exam. Now last year 2022 I decided to go and find a dental surgeon to remove my wisdom teeth. Had it removed and healed. Months later, the same root canal tooth had been infected again. 6 months ago I went to her and she said it was the surgeons fault that the tooth is infected. I decided to have a second opinion and chose a Dentist of my choosing not her so called referral. I went to a different dentist and had it checked and was advised to have it extracted due to a recurring infection and a cracked tooth shown in the **ray . Today 06/29/2023 I have the root canal tooth ****************** refused to help me with pulled by a dentist of my choosing , the root canal tooth has a remnant of a metal on it and a tooth that is cracked because of ****************** actions. She had caused me a lot of pain, patient blaming and she has a lack of accountability and refuses to help me in my time of need. This Dentist needs to checked by the board of dentistry because of her actions. I know Im not the only patient who has encountered a misdemeanor.       My son had a dental appointment on January 12, 2022 where ****************** said he had 8 tiny holes due to the glue from when they removed his braces. I then called the Orthodontist to let them know and they called ****************** and she has now changed her story to say that she never said that. She is saying that he has decay from not brushing his teeth. My son has not had a cavity in years and he goes to see her regularly (every 6-8 months).

      Patient *************************** came to the office for prophylaxis, exam and x-rays and upon examination, class V lesions on the cervical 1/3 of the labial surfaces of incisors, premolars and canines were noted.  Patient just had his orthodontic braces removed and I even called patients mother and showed her his poor oral hygiene which was the cause of the cervical lesions on the teeth.  **************** which is the mom misunderstood me when I said I also needed to remove some excess glue which remained after the removal of the braces at the orthodontists office which is normal but never indicated that this was the cause of the class V lesions from the glue.  

      The patient last prophylaxis appointment was 1/12/2023.  He had generalized moderate gingivitis that causes irritation, redness and inflammation that can cause cervical lesion or cavities if proper oral hygiene is not maintained. Radiographs and photos are attached.  We never failed to give proper oral hygiene instructions to the patient.  Patients last cleaning before the one in January 2023 was ***** 2022.  Patient has had caries in the past and fillings on eight of his teeth back on 4/5/2021.
