I purchased services from Affordable Burial & Cremation Services, Inc on 6/16/2022 to have********** ******* ******* cremated. 2 things i noticed on the Death Certificate was that on line 40. Place of final disposition was a name and address that was not mines and the cause of death was incorrect. So after reaching out to the doctor and multiple calls later i was advised that the death cert. was going to be amended that i will received the amended death cert. by mail. I received the amended cert. today in the mail and the only thing that was corrected was the line 40. Place of final disposition however the diagnose was not corrected and on the receipt where it says responsible party has someone else name that i don't even know of. It has been really hard to getting things straighten out with this company and they are very unprofessional.Initial Complaint

July 18, 2022 Affordable Burial and Cremation, Inc. ************** ***** ****** Owner, ****** ***** Director RE: ******* ***** ****** - Deceased Relative I used Affordable Burial for my Mother back in 2018 when she passed away. I was happy to have a Mortuary I thought would handle my current loved one, ******* ***** ******** funeral arrangements. The day I called regarding *********** I was transferred to *********** to handle all aspects of my current mortuary needs. I explained to her that my loved one had died of COVID-19 on February 28 and was located at*************************************************. I also explained to her that I wanted to have*********** cremated and to have his ashes scattered at sea. In the same conversation, I requested two (2) copies of the death certificate. Several calls have been made after March 11 to follow up on the process; just to make sure things were moving towards a positive conclusion. In April 2022, I was told that I could come and pick up the separate small Urns and the Death Certificate. When I arrived at the Affordable Burial and Cremation's Mortuary I was given the small Urns and I was asked if I wanted the rest of the Ashes. I informed the staff member that the bulk of the Ashes were supposed to be scattered at sea per my original request with ***********. The staff member checked the file and told me that it was not stated in the file that the Ashes were to go out to sea. I promptly asked why I would order four (4) small Urns of Ashes separate from the larger box of Ashes when I picked up the Urns. I repeated that the bulk of the ashes were supposed to be scattered at sea. Finally, on June 23 when I called again, I was told that ************ Ashes could not go out to sea because there is an additional permit fee that has to be paid. Please investigate this ****** **** is the Director, and the owner, *** ***** ***** has little regard for how bereaved family members are treated.Initial Complaint

Chanel & Pat at Affordable Burial & Cremation has refuses to send me my mothers death certificate. My mother passed on **************. As of today, *** *** ***** I still have not received it. I have requested it my email and phone three times each. I have also complained to the Cemetery and Funeral Bureau. I contacted another mortuary in ********** and they said all of their death certificates for Oct, Nov and Dec have been processed and sent to the families. Can you please help me with getting Affordable Burial and Cremation to cooperate with sending my mothers death certificate to me. Thank You
