On May 10, 2023 I visited C***** Health In *******, **. During my visit I used my FSA card to pay for the co-pay of *****. That payment went though fine. After my visit, I was advised that Carbon Health no longer takes my insurance directly, so the office asked if they could charge my card on file, my FSA card, for the remaining balance of $******. I gave them the authorization to charge my FSA. FSA then reached out to me to submit a bill or receipt to authorize the transaction on their end. I went ahead and forwarded the informaiton needed and my FSA approved the amount and paid the ******. The amount was then deeducted from my balance. A few days later, Carbon Health contacted me advising my FSA declined to the request. I reached out to my FSA company who then supplied me with proof of the transaction going through as completed and confirmed that the ****** was in fact deducted from my account balance and paid in full. I supplied this information to Carbon Health and as of today, 09/07/2023, Carbon Health is now trying to send me to collections for the ****** bill that my FSA has already handled and paid in full.
