    On 5/3/24, I presented to this office for emergency dental services due to my dentist office being closed and my future appt on the 13th being far away to handle a swollen face and toothache. The office accepted my dental insurance provider and offered me a same day appt. Dr. **** and his office were great and helped me relieive my pain. Before I can recieve any treatment, Dr. ****** accountant who handles the fees ******** had me sit with her to go over the tratment plan that included future extractions of my wisom tooth and the infected tooth for one week later after the incision and drainage of my abcess. She quoted me $4100.00 for the service and this did not include a crown nor implant. At the time, I agreed to the plan because I believed that this will help me relieve my pain and make me feel better. Once I signed the paper work, she assisted me in opening a care credit account to cover services rendered for the day and the future scheduled date for the extractions. During the weekend, I had a chance to review my paperwork and evaluate the upcoming appt, after careful thought I decided to follow up with my family dentist to do a treatment plan. I called the office and left a message with the team to cancel the upcoming extractions that was scheduled for Friday May 10th via voicemail, on Monday may 6th I recieved a message from the team that my appt was cancelled and $3,337.58 would be refunded back to my care credit account, but left with a charge of $746.00. I signed the refund slip, however I asked for a breakdown of charges for the $746.00 that were charged to me so I can have a itemized receipt for my records. It took many voicemails and many e-mail messages to get a response from ******** telling me that my insurance only paid $66.00 for some of the x-rays taken. According to my insurance Pearl submitted a claim for $350.00 for X-rays and were paid. Please assist me in getting a itemized receipt, the office has been unresponsive. Thank you
