      Please see details of the previous communications from 4/24, 5/2, 5/16 below. None of these have resulted in any response from the company, whether submitted via the website or emails. There are too many characters to include all follow-**** but I can share them with you separately if you require them. The doctors name is located in the first mail below.       We are very disheartened at Continuum that the process for resolving this death certificate change request has been so protracted. We received the initial request for the change in cause of death on 4/28/24 from patient's daughter ***************. I called her and spoke with her, telling her we could certainly have the cause of death changed. Our Admin Assistant immediately contacted our MD, ******************, who responded that he would of course change the cause of death as family requested. Our Admin requested from Tulip Cremations, the mortuary, a new death worksheet which is required to have the cause of death changed.  Our contact at Tulip Cremations was/is ***************************. There has been a lengthy delay in communication from the mortuary to Continuum. Our Admin contacted ***** again, only reaching VM and leaving requests to call us back. I called daughter **********;  again, I do not have the date, but left VM to assure her we had not forgotten this and would continue to follow up until it was resolved. On 5/29 I again left another VM for daughter that we were in holding pattern with the mortuary but would continue to pursue this to completion. I have left my cell phone number for daughter ***** and encouraged her to call me at any time of the day or evening to discuss, but she has not called me to date. Today, June 5th, our Admin has again contacted ***** and reached only VM. As soon as we can get the new worksheet form, which we hope will be today, our MD can make the change in cause to death to specify only "Congestive Heart Failure" as family would like. The worksheet with the changed cause of death is then sent to Tulip Cremations, and they send it to the state. We are committed to seeing this through and will ensure that the cause of death is changed as this family has asked.
