For the account of ***************************, a $121.84 refund for the following 4 office visits is due. The payments before service, by myself to ****************** office totaled $663.27. After insurance credits and patient responsibility limits, the total patient responsibility was $541.43 for the same 4 visits. A refund in the amount of $121.84 is due. ****************** office has not responded to written letters and documentation regarding this matter. The timeline of these visits is extended, dating back to Feb 2021 through May 2021. This is due to COVID-19, insurance processing timelines, ****************** office holding funds in case of future visits, and delays due to the lack of response from ****************** office to my requests. ****** is not an active patient of ************** at this time and there is no reason for ****************** office to continue to hold a balance of funds, indefinitely, for future office visits.
