      --11/17/23: Visited Affinity Dental, Hillcrest for dental work. To my surprise, they said I no longer had insurance with Liberty Dental. I said I had paid my yearly premium. I decided to proceed with dental work, then contact LDental later with proof of premium paid, and get reimbursed from ADental for out-of-pocket expenses.--11/20/23: Wrote LDental with copy of premium payment check to them. Check# **** in the amount of $233.56.--12/07/23: Liberty Dental wrote they had made mistake and would reinstate dental insurance."...the office (Affinity Dental) will reimburse you for out-of-pocket fee you paid for..."--12/15/23: I emailed Affinity Dental asking when I'd get reimbursed for out-of-pocket expenses incurred. No answer.--1/05/24: I wrote a letter to ********************** of Affinity Dental asking when I'd get reimbursed. I included SASE. No answer.--Sometime later got voice mail call from ******** of ADental. She wanted my address for the reimbursement check. I used their web site's email address to give my mailing address. Still no check.--2/04/24: I left message with AFinity Dental. Said I still hadn't got my check. Again left my mailing address. As of 2/08/24, still no check. I'm not the only one who has had problems with Affinity Dental, Hillcrest. Others have filed ********* BBB complaints against them. They've also gotten poor reviews on Yelp. I've filed a complaint also with the ********** ************************* and will file with the ********* *************************** ADental owes me, according to ********'s voice mail (WHICH I HAVE SAVED), $174. I'd be happy to send you hard copies of all my documentation, including: the email sent to ADental plus the letter sent to ************ Koryurek plus Liberty Dental's letter to me admitting they were at fault in not crediting my premium payment.

      I had an initial visit on 5/11/23. I was quoted prices for services and was then sat down with an employee to discuss costs. The employee pressured me to apply for a CareCredit credit card online for several minutes, and took the information I had provided for the appointment and immediately went to the credit card website, filling out my information without asking me. I asked if this would lead to a hard credit inquiry, which another employee confirmed it would, and I adamantly declined several times for her to apply. Yesterday I received an email from ************** stating an application had been submitted for the credit card the day after my visit on 5/12/23, without my knowledge or permission. I called and filed a fraud complaint with Synchrony, and left both a voicemail and text message to the business, which has not reached back out to me. I also asked for an itemized list of the expected costs I was quoted for future work to be done, which I was told would "take too long" to create that day but that would be emailed to me the next day. I never received it. The prices are also extremely high for having insurance coverage. I now have a hard inquiry affecting my credit, and was told it will take 90 days to be reversed.

      In summary, what was supposed to be a routine filling ended up causing permanent damage to the root of my tooth. I am now living with extraordinary pain and a stubborn oral infection. The only solution is extraction of the tooth and replacement with an implant. H32**3238**38343635H H333835**343236363939H will not claim responsibility for destroying the tooth, even though it was perfectly fine the day before the filling. I cannot afford the procedure on my own, so I continue to live with the pain that they caused for almost a year.Details below. Additionally, I attached a very detailed timeline. X-rays and H333835**343236363939H records are in my possession and can be provided.3/17/22, I went in for a **routine filling on tooth #**. 3/18, I woke up in extreme pain. 4/19, After several return visits where they continuously denied any faults or wrongdoings, an alternate dentist at the office finally discovered an issue with the initial filling, and agreed to re-do it. An appointment was scheduled.4/20, In terrible pain, called office to ask appointment be moved ASAP. Received no help nor compassion from front desk. Was interrupted, screamed at, hung up on and generally gaslit. 2 days of calls, no help.4/26, Filling was redone and the pain subsided. But the tooth hasn't felt right ever since.9/6, Routine cleaning. I asked for the tooth to be examined. Hygienist cannot examine, and claims nobody is in the office to do so 11/**, elective exam appt, discover cracked route and developing fistula 12/2, Endodontist confirms tooth is damaged beyond repair and must be extracted.Continued fights with the office to help compensate me for the necessary repair procedures, as the damage was so clearly caused by them. They continued to deny responsibility, claiming I am at fault due to bruxism. This is false. 12/22, lawsuit is filed with attorney but does not go through **2 Dentists have since examined my X-rays and determined the initial filling was completely unnecessary.I also contracted Covid here

      I went to get my teeth fixed at Affinity Dental of Hillcrest. The lady I talked to said that It would probably be best to remove all the teeth and use dentures. So they removed about 17 teeth and made some dentures which cost about$3600. The dentures do not fit and they don't want to fix them so they fit. They get upset when I tell them they don't fit. They also want more money to do any more work.. I believe the deal we made was that they would fit so that I could eat using dentures. Not even close. The last time I went to the office the doctor told me the only way to eat was with implants. They should have said that from the beginning. Implants cost at least $30000. I would have made different. decision.

       We are sorry to hear what is going on with our patient. Patient was recently seen on  Dec, 1, ************************************** order for a better fit he needs what is called a denture reline at $0 charge to him since his insurance has benefits remaining to help him pay for services rendered. We did reach out to patient yesterday 2/22/2022 and left a voicemail since there was no answer to follow up procedure and cost. Patient did call back to confirm and scheduled an appointment. Consents are signed by patient informing cost and follow *** that are required.
