Cucamonga Valley Medical Group provided incorrect codes for labs that were taken by Quest Diagnostics on 11/10/2023. I am now being billed due the claim being denied my insurance. My insurance, Quest Diagnostics, and I have contacted Cucamonga Valley several times to obtain the correct codes with no response. Every time I call, I am told that I will receive a call in the next 24 hours which has not happened. I was able to initially put a 45 day on the bill from Quest to give Cucamonga Valley time to remedy the situation, which they have failed to do. I have now received the bill for a second time, and there seems to be no urgency with this facility. Not only is my health being affected, but if this is not taken care of immediately it will affect my credit and my job. I spoke to a lady from Cucamonga Valley this afternoon, and by the end of call she seemed very rude and annoyed by my request.
