I want my health records covering 24 years.I saw *********************** for about 24 years (2000-2023). First he was in private practice. Then he became part of Rocky Mountain Urgent Care and Family Health Medicine.Oddly the phone number rarely worked for this company and the facility was sometimes dirty.But my complaint is RMUC went out of business with about 2 weeks notice in early 2023 and didn't tell the patients. Now all my health records are lost. !!!! Basically I want my health records. I want Delivery of my health records. I wrote them as instructed by a receptionist before the phone system shut down permanently.Then I received an email this month with a non-working number. Was sent here as they took Medicaid...they didn't enter my insurance correctly and told me there was an amount due. I told them they did not have both insurances on it. They told me they did it right. I showed them the form that only one was listed. She said she fixed it. I received a bill in Nov for $300, I have been calling 1-2 times weekly since then to try to resolve this. When I talk to someone they promise to call me back, but most of the time I have to leave a message or am hung up on. I finally paid the bill because I do not need collections coming after me for their negligence. They need to do their job...enter information correctly and call back. Their staff was very unpleasant and had my child in tears after going in for a strep culture.Initial Complaint

This is for the Rocky Mountain Urgent Care on ********************* (***************************)I was billed for labs from Quest Diagnostics that could not be explained to me. I had an office visit (that I was also charged for) and the provider did not complete the labs requested but I was still charged. They didnt run my insurance and therefor Quest billed me for labs. They dont respond to my messages. I was finally able to get ahold of a rep and she told me to give her 2 days to sort the issue out. That was 2 weeks ago and this has been ongoing since June 2022
