Service provided on 7/4/22 at AFC Urgent Care. I was billed *****. However, my insurance company paid AFC ***** in August 2022.I have been waiting since August for a refund of *****.I am including documents 1) email from AFC telling me that they will charge my card, 2) EOB from my insurance (on page 6) showing that they paid the charge, 3) my credit card statement showing that I was charged.Business response

This patient Is In fact due a refund from our facility. There was a glitch in our system which
posted that the refund was due to the ***************** Instead ofto thepatient, making it
un-searchable when we run reports monthly for patient refunds. This has been addressed immediately
and a refund check promptly sent in the mail. If there are any further needs, we can be contacted
directly and would be happy to assist quicker.Customer response
