    I had one visit at **************** Interventional Pain Mgmt under the care of Dr. **** on 12/28/2016. The procedure was preauthorized with UnitedHealthcare ******** before they would even schedule an appointment. They filed two claims to UnitedHealthcare for the same visit, same day, same procedure, and same amount. The first claim was filed on 4/11/17, processed on 4/17/17, and shows that it was entirely covered and I owe $0.00. The second claim for $1,451.00 was filed on 5/16/17 and was processed on the same day. It was denied since it was a duplicate claim for the same visit that had already been covered. I attempted to sort this out directly with CS Inter. Pain Mgmt thinking it a clerical error. They directed me to their billing service who refused to listen to anything I had to say about the matter. I tried explaining I only had one visit and my insurance received two claims with the same date of service which was ignored using the excuse that the billing file numbers were not the same as the insurance claim numbers, I suppose using my name or birthday to look up my records must not work either. The woman I spoke with from billing became hostile demanding I was at fault for the balance on the duplicate claim. As an attempt to collect money from this duplicate bill, they adjusted the amount to $293.44.At this point I reached out to UnitedHealthcare to report the incident and was assured repeatedly that I did not owe them any money for that visit. UnitedHealthcare sent a letter to the doctor preforming the service, Dr. ****, informing him that I did not owe C S Inter. Pain Mgmt money and I was sent a copy of that letter.I thought it was handled until it showed up on all 3 of my credit reports this month and a new bill came from debt collector ******************. demanding I pay the $293.44 plus $135.90 interest with the threat to accrue interest daily. This false debt should not have been sold they need correct this with ******************. and my 3 reports.
