Medsurant, LLC. is charging us for services rendered that were not authorized under normal processes and explanation. My wife went in for back surgery on June 3, 2022 in *********, ** at ************************ and completed the prior approvals required by out insurance company. On the day of the surgery we completed the registration process and confirmed our insurance carrier Aetna had approved the surgery indicating it was designated as "In Network" and covered. We received our first bill from Medsurant over a year later some time in July 2023 for $9,631.00 to our surprise. When I called the company for an explanation of the bill I was informed it was not covered under my insurance plan and was "Out of Network". When I asked why it took so long to receive the details of the bill I was told the company was working with the insurance carrier and had appealed it multiple times trying to get it approved and covered but failed. When I asked for proof of our approval for the services and billing I was sent a copy of the document signed by my wife. I asked when was this document presented to us and explained to us that it was not approved by the insurance carrier. I was told it was not known at the time of the surgery if it was approved and was also informed it was signed by my wife in a patient room just prior to the surgery. Knowing this it is clear to us that we were misled by Medsurant because the procedure was not pre-approved by our insurance company and we were not given the opportunity to reject the services since they were not required for completing the operation but desired by the surgeon per his request. We are requesting that this bill be written off by Medsurant because of the deceiving manner in which the approval to proceed was granted and that a prior approval was never submitted or explained to us resulting in a ridiculous bill over a year after the surgery.Initial Complaint

I received a bill I believe to be a scam/fraud. It states I owe $610.30 and threatens collections. This is the first time I've ever seen a bill from this place before now, if it is legit. The date referenced was over six months ago! The insurance company is wrong. I've never had **/BS. There is also a secondary insurance. Calling the phone several different times always goes straight into a man's voice mail, not a ********. A company name is never mentioned. A company operator never answers. I used several databases and can't locate Medsurant Health or Evokes. I called the hospital and spoke with Billing and Patient Records. They don't have record of that doctor and have never heard of Evokes. They need to contact the hospital to get this straightened out. Claim number listed: *************
