The business Appleton Clinic charged my credit/debit card after being told that the charge was not authorized by me. A member of Appleton Clinic (****) contacted me on November 21st 2023, in regards to a balance. In speaking with ****, I disputed balance. This balance was accrued well over a year ago (and probably much closer to 2 years). The charge was from a laboratory strep throat test performed at Appleton Clinic. I was told at the time of testing by an Appleton employee that the test would be covered by the health management program/ insurance that I had been paying out of pocket on a monthly basis. **** said that he would present my concern to the "team". I have no idea who this team was to see if they would be able to negate that charge. I very specifically told **** that I was not authorizing that payment. As a matter of fact, on November 21, 2023, **** was told multiple times that I was not authorizing that payment. **** assured me that my credit/debit card would not be charged. November 22, 2023 my credit/debit card was charged $40.00 for payment on that balance. Unfortunately, this is not the first time that Appleton Clinic has miscomunicated to me about what this health management program/insurance plan covered. A prior negative experience cost me approximately $1500.00. Sadly, I was unaware that the $40.00 charge posted to my account until today, December 4th 2023, because I TRUSTED Appleton Clinic to keep their word. While $40.00 is not a large amount of money, it was the only money I had available to me for fuel costs. I called Appleton Clinic today, December 4, 2023 and spoke with another representative named ****** (I think). She assured me that the business manager would be returning a phone call to me before close of business today. That also did not happen. I have started the process with my bank, ******* ******* ****** *****  to dispute the $40.00 charge. Thank you for your time, attention, and consideration in this matter. Respectfully.
