My daughter went to the *********** location between Aug 2023 - March 2024. After they ran insurance they said we only had a $45 per visit charge, which we paid each time. We later received a bill for early 2024 visits (at that time we still hadn't received anything for Aug-Dec 2023) and it was a very high bill. Upon calling they said "Oh that's just the co-pay and you signed saying you knew it would be more." We don't even have a co-pay with our insurance, which is why that was very confusing and not transparent. I let that go because maybe I signed something but I would like to have taken their word on charges when we originally ran insurance. If they had responsible billing practices we would have known the increase far earlier and potentially would have made other arrangements. Why ************ 2024 before ANY of 2023??? I have now requested MULTIPLE times to receive a full statement that includes all in-office payments as well as all online payments, yet they STILL only send the in-office payments and charges on the billing statements. I can't reconcile any of this without seeing the full history. I have called multiple times and recently filled out on online form requesting this. I don't think my request is unreasonable. Show me the full detail of ALL charges and ALL payments made both in-office and online. They should have that information. I am being completely ignored. I am fully expecting them to try to send this to collections but I have proof of phone calls and I recorded my online submission that I have attempted on multiple occasions to request what should be really simple for them to provide. It's giving sneaky billing practices. Money isn't the issue. The lack of honesty and transparency is. Just send me an all charges/all payments history so I can review. Why are they withholding this and making it difficult to see? I will hire a lawyer if they attempt to send to collections. Just give me the FULL statement. Upon receiving the bill for physical therapy services from Rocky Mountain Spine & Sport Physical Therapy, I reviewed the services and codes they had entered and feel this is a deliberate over billing. During my ************************************************************************************************ are written so ambiguously they can claim just "asking me to stretch at home" as an activity for which they charge an additional $60. They will claim my sessions were an hour long, but neither of them lasted more than 45 minutes. This is an important distinction considering I don't believe there was enough time in the session to satisfy the time for which they have billed.I have spoke with my insurance company, with the Rocky Mountain Spine and Sport billing department, the physical therapist (*********************) and ******, who I believe was a regional manager. Variations on the same excuses arose, but felt they were simply obfuscating and depending on semantics for justification for each line item on the bill. I am seeking a reduction in the bill to what would be fair. Having already paid $90 as a co-pay, it is fair to pay an additional $150. This brings the total to $240, or $120 per visit.Initial Complaint

My husband *********************** had a stiff shoulder at the time I left overseas to attend to the unexpected death of my brother.The doctor prescribed physical therapy and had him go to Sport and Spine Physical Therapy. The first visit he asked for a predetermination from the insurance company so there would be no surprises. Sport and Spine Physical Therapy told him they contacted our insurance company it would be $45 a visit, which he paid.Once I returned from overseas, we received a bill for over $1126.96.Taking a closer look at the billing, each visit runs $250-$400 for a short visit with multiple procedure codes. We disputed the billing with the insurance company who just said the procedure codes are valid, with no proof being performed. My first call was on Thursday July 8th 2021 asking for explanation of this large bill when we were told it would be $45 a visit, and what the multiple procedures per visit were. We requested copy of his medical records which we have not received as of today.I mailed a letter 11/7/21 and check #**** for $220.00 again disputing procedures that were not provided and for medical records and did not get a response.According to the state law No Surprise Act law of 2020, there should be no surprises on medical billing.Since 7/8/21 I have made multiple phone calls, emails and written documentation throughout 2021 regarding services not provided but billed for. I have spoken with ******* in billing, ****** the office coordinator *************) at centennial and the last contact I had was with the Area Director ******* on 12/15/21 who said she will have our medical records mailed to us.We were asked to send the ***** release form twice, sent via email 7/9/21 and again 12/15/21.Records have NEVER been received. Therefore we would like our $220.00 returned and/or our account settled.Thank you. ******* and ***********************
