      On July 14, 2023 I had my routine 6 month cleaning performed, during this exam I was told I needed antibiotic injections due to some buildup. At NO time was I advised it was or could potentially not be covered by my insurance. Fast forward to my filling appointment on August 26, 2023, upon check out I went to pay my quoted amount of 81.00 and was broadsided by a large bill from July's visit. I had never received a bill so this was a shock to me, so much so that Danielle the receptionist could see me visibly shaking at the news. I paid the amount due for that day and asked to allow me to call my insurance the following Monday. Danielle also advised she would follow up with her supervisor due to the circumstances of me not being informed that these antibiotic injections were not covered. I called my insurance the following Monday, August 28th and was told to have them send to medical for processing. Today, September 15th I received my FIRST bill for the July balance. I immediately reached out to Danielle at Dental associates who advised medical processing was pending and to disregard. This proved untrue as medical has not been billed for the July date of service. I was also apparently overcharged for the August date of service as per my EOB I only owe 51.00 while I paid 81.00 - the overpayment has not been brought to my attention and had I not gone to review the claims in July with my medical I would have never known a refund for the difference was due to me. I am horrified and have attempted to resolve this respectfully however I am not being provided with truthful information. The billing department should really be investigated. I am at a total loss over the lack of information provided as well as the lack of truth in any information I have been given. I have never authorized services to be performed that were not covered by my insurance. This is now the 3rd billing issue with this office, the first was misinformation for coverage and an estimate for my spouse.

      I went to Dental Associates as a transfer patient with orthodontia on my upper and lower teeth (second bicuspids to second bicuspids). Nearing the end of my treatment I began to point our problems I would like to have fixed by Dr ****, spacing, teeth which needed to be straightened. He said they would look straight after the braces were removed. One specific example upper left lateral incisor. It is still not straight. There is also spacing between upper second bicuspids and first molars where food becomes impacted. On the right side my teeth fit well together on the left the teeth do not fit together at all and I mentioned the left side bite on at least 3 visits. In addition the diastema between upper central incisors remains. I do not have final records as is customary upon starting and finishing orthodontia. I can take pictures to support my concerns I was only left with 2 panoramic xrays and 2 essix style retainers. In addition my teeth are not in an ideal class one bite. If this could not be done it should have been stated at the initial consultation. He also said the teeth would settle in. They are settling apart. I have gone back to fix some corrections in my bite which resulted in filing down teeth. The way my teeth are currently, they will wear unevenly and currently cause discomfort on my left side. I will need to seek more orthodontic treatment to fix the problems listed above, for which I am requesting a refund to put toward orthodontic treatment.
