      Dear Sir/Madam, I have been paying storage fee for my embryo to CARS (Center for Advanced Reproductive Services: ***** *********). Last time, I paid my storage fee $160 for the 2nd quarter of 2023 (please see the attached email as proof). The $160 covers the whole quarter. In the past, they told me the fee is pro-rata based on the days, if I decide to dispose of my embryo, I should receive a partial refund. Then, I decided to dispose of embryo on 6/10/23. Please see the attached email as proof. Then, they told me requesting disposal online using myself' account was not enough, and asked my husband and me to sing the form. They made the signing process extremely difficult intentionally, in the end, my husband and I had to show up in person in their office at 2 Batterson Park Rd, Farmington, CT 06032 on 6/17/23. There was a witness from the lab when we signed the form. Since the $160 covers the whole quarter and I disposed my embryo on 6/10/233 and 6/17/23, I should be eligible for a partial refund (if using 6/10/23 as the last day, I should be able to get: $160/3=$53.33 each month; $53.33/30 days X 20 days= $35.56; if using 6/17/23 as the last day, I should get $53.33/30 days X 13 days=$23.11, at least). Although the amount isn't that high, but that is my money and I should be eligible to get them back. They should have refunded me the money automatically, but they never did it. Then, I called the billing department at 888-540-6619 multiple times and talked with different people, including Cindy, Annette, etc. So far, I still haven't got back my refund. I know they have been holding other patients' refunds (who are in the similar situation) and never refunded them, very Bad! Maybe this should be disclosed on social medias? Please note, CARS (UConn Fertility) works together with Embryo Options for the embryo storage. That is why both of their names show up on the email. Thank you so much for your time and help!       I was in this facility a few weeks ago to update my account information. Upon trying to use the ladies’ room I was approached by an employee who I now know is in charge of this facility. He was shaking (adrenaline rush) and attempting to ask me questions and enter the room or stop me from entering it. I recorded this individual as he walked into the office area and left. I then received an email stating I am not longer welcome to do business there and should move my cryopreserved sperm or they would be destroyed.

      ********** arrived at 7:05am on Nov 16th, parked in no parking space by loading zone.  She attempted to enter through and employee entrance and then entered first floor lobby.  She inquired about storage payments due with a 1st floor patient care rep.   After given information, she went upstairs. During the exchange on the first floor the individual was acting strange, and patients also reported she was carrying a knife.   911 was called.    ********** proceeded to the second floor, continued to act strange, tampering with a bathroom door, and making staff very nervous.  ********** left the building after being confronted by a senior staff member.  And she left before the police arrived. 

       

      Police arrived and one of our staff provided information and gave details above.  The officer contacted her on the number provided and she was belligerent with him asking why he was calling her.  He informed her not to return to the building

       

      Our team is now notified through our EMR to call 911 if ********** returns.  

       

      A full incident has been on file at our clinic.

       

      Due to her behavior, and a violation of our policies, I proceeded with a “termination from CARS” letter.  We provided resources and information for ********** to continue care elsewhere, we also provided specific info for alternative cryo storage facilities, but at no time did we state her cryopreserved sperm would be destroyed.

       

      The description of what happened (as detailed in **********’s complaint) is far from accurate.

      To whom it may concern,

      the Business has not been open-minded or honest about their responses and anxiety surrounding a holstered pocket knife. Their attempt to insinuate that this knife was in the convicted party’s hand is egregious. While checking a bathroom door to understand her safety, ********** was approached by a man (not a woman) while trying to use the restroom. Her records clearly indicate her as a trans-gendered female, and this incident and the surrounding perspective of the outlying community of Connecticut seems to accept and support the ideas of hate-crime and discrimination. ********** possesses video evidence of this transgression, and would like an apology from the center for advanced reproductive health, admitting they are backing the alleged, yet false story of Scott, a middle aged man that decided to attack and accuse her out of his obvious mental health problems, anxiety etc. resulting in a hate crime in the face of all of Connecticut and the United States of America.
