      Jan 07,2024 I had gone to the local medical walk-in in regards to having dizziness in which had been diagnosed as vertigo. upon arrival I had been asked by the receptionist if any insurance changes and I stated no. a portable laptop had been given to me to complete registration. I had been told that if I owe a copay they will contact me. I had not been offered any assistance in completing the registration even though vertigo was occurring. I patiently waited for my name to be called. I had been in the facility for not even 10 minutes for doctoral observation and a rx was sent to stop and shop. the physicians assistant felt vertigo was the condition I had been experiencing and if he felt differently he would send me to the hospital but felt comfortable with his decision. I had gotten my rx filled and went home. no follow up calls from the walk in were made the following day in regards to my health status. I just received a balance due of 666.00 in which I feel is unethical. I had contacted the insurance company and they stated that I had gone to the emergency room. this is a walk-in facility I have used on many occurrences for family etc.. and never received billing as such. nothing was ever mentioned to me in regards with my insurance and was once again informed that if the diagnosis was more severe then would be sent to the hospital. this facility is billing as if they are a hospital and it was clearly stated to me that I would be sent to a hospital if diagnose was different. I feel the company is just coding such visit as unfair and I am kindly asking for an internal review and proper billing to be completed. Yale has a 2.5 rating on overall review. honestly I am extremely disappointed in there actions and will vertically not return to this facility. this just makes me question the fact. where does one go?? I've paid health insurance respectively for 40 plus years and now this ?????. Yale billing had broken down payments for 83.25 for 8 months if I am unable to pay 666.00 up front. either or is way out of my budget especially when I generally have a 40.00 co-pay. please address. thank you! ***********************

      Despite clearly communicating my intention for an annual physical exam during the appointment scheduling and the visit, the doctor insisted on classifying it as a medical visit. Our discussion revolved around my family history and a mention of potential hypertension in the future, with the aim of preventive care—consistent with my experiences during annual physical exams with other primary care doctors. Charges for such discussions have never been incurred in my previous annual physical exams, making the unexpected change to a medical visit with associated charges both surprising and concerning. The shift from an annual visit to a medical visit happened without prior discussion or my consent. Despite my efforts to address this matter with the doctor's office, I have been informed that the service was performed regardless of my request. I want to stress that I neither requested nor authorized the specific service in question. I am disputing the associated charges totaling $959.15 ($282.76 for the office visit + $676.39 for lab tests). I reached out to my healthcare insurance company, and they advised that the doctor only needs to correct the code to the annual visit code or submit both the annual visit code and the medical visit code. Despite my requests, the doctor declined to make these corrections.

      I am being sent around in circles. I have clearly established what my problem is. I am directed to the ******* system and then sent back and around. I have proof of my problem in sleep studies. I want proper care. Sleep Apnea was first diagnosed by my heart doctor. If I go to the hospital for a heart issue I am seen immediately. For sleep apnea I have been stalled for 3 or 4 years. I want this probelm addressed as fast as you would for a heart issue. I do not want to stroke out waiting. Sleep apena has cause diabetes, heart disease, vasular probems, and ruined my life. I am not waiting any longer I want prompt care.

      Paid for my office visits. In return, I need my prescriptions filling accurately and in a timely manner. There have been instances in the past where my prescriptions were sent to the pharmacy with an incorrect dosage and/or quantity. I made clarified the type, dosage and units of the medication but still needed to call back several times over several weeks without resolution; meanwhile, I am out of supply of my medication. Most recently, on 11/3 I went to *** to pick up my medication and my doctor Joseph R*** called in the wrong medication and/or made changes to my medication contrary to our prior conversation and without consultation. I notified his office on 11/3, 11/6, 11/7 and 11/8 but did not get a resolution. He finally called me on the morning of 11/9, stating that he fixed my prescription but was expelling me as a patient becuase I wasn't happy. I reinterated that I was not happy with the errors, lack of communication and failure to respond in a timely manner with corrective action. He said he would take care of my prescriptions for another two weeks. When I went to the pharmacy to pick up my prescriptions, he did not correct my prescription. The pharmacy sent the correct request on 11/9 and as of today still not filled. I contacted Yale New Haven health patient experience on 11/9 and they said it would be resolved by 11/10; it was not resolved. I called the on-call physician on 11/11 but they did not return my call. I called patient relations today and they still have not followed up. I am going on my 3rd week without my medication and I feel like I am being discriminated against. I would like a follow up and resolution from upper managemnt.

      I have been receiving mail from Yale NewHaven Health for "******** ****** or Current Resident". ******** ****** no longer lives here, please stop sending mail to my address as I do not give consent to receive any type of mail from Yale NewHaven Health. Please take my address off. your list and do not share with any other parties. Please provide confirmation that my address has been removed and that I will not receive anymore mail from Yale NewHaven Health. I attached a photo of the mail I have been receiving for reference.

      Received unsolicited marketing mail (spam) publication 'Advancing Care' which I never signed up for and never requested, during March 2023, and April 2023. Requested removal from the mailing list via email to stated email address on said publication, ****************************, during March 2023, and never received reply, and received further marketing mail in April 2023.

      They have billed the wrong insurance over and over and I have uploaded the cards into the Yale chart. I had Aetna from September or October 2020 until January 2021. I had husky A at the same te as always. I had blue cross blue shield in Feb 2021. Still had husky A. I don't know what is going on but since covid19 it's been a mess and now they leave me with this bill I don't think so. I myself have been out with covid19 out of work and I do not have this kind of money to waste on something that was not my mistake.

      I visited the ********* ********* ****** in ********* thats a part of Yale New Haven Health system as per their employee Karen. I made regular payments to them and they cashed a check payment from me last month only to send my bill to a collection agency. I spoke with them and my credit will be effected if I don't pay the bill off. I tried to contact Yale but they seem to not be on the same page whether ********* is even a part of their system and have thus washed their hands of the issue. ********* is hiding behind Yale and Yale is ignoring my billing issue. I need this resolved as I have paid according in good faith with the payment plan originally intended.
