      On September 24, 2023, I visited JFK Medical Center located at ***********************************************************, for a checkup. I had cold symptoms, and this facility came up in my ****** search as a no-appointment-required urgent care center. I have visited similar medical facilities in the past and the charges for services were $50. I was shocked when, the following month, I received a $2,000 bill. Apparently, this facility is associated with a hospital. However, nowhere in the description was it mentioned that I was visiting a hospital. The facility looked like a regular, small, urgent care room that is located in a normal shopping plaza with a Publix beside it; there is no hospital in sight; there is no signage or inclusion in the name of the facility indicating "emergency room." My vitals were taken, I received prescriptions (for the wrong medications - after explicitly and thoroughly explaining my allergies to said medications), and this is all. After receiving the bill, I have contacted the provider at ************** and was told that my bill will be re-coded. However, the bill remained the same, no adjustments/re-codings were made, I was sent an additional bill, and I was sent a request for all my personal information including social security number and tax forms in order for "financial aid to be considered." At no point was an explanation of charges provided to me or my insurance, despite multiple requests. The billing department cannot provide an explanation or way to contact separate departments for multiple charges from different sources from the same facility. The practices of the facility are misleading, invasive in terms of privacy, void of personalized health care (issues regarding allergies and medication conflicts completely ignored) and the the customer service is non-existent (the billing department will provide no explanations or help to either customer or insurance agencies). The bill should be revised.

      Jan 3rd I went into JFK ER now called, HCA ** ************* Emergency, for an infected mosquito bite. They took $400 from me immediately before seeing me. I was seen by Dr. *************************** who worked there. He performed services and sent me home with antibiotics. They took blood work. I was called the next day to go back in for another blood test. They said my blood showed bacteria and that I could die? I felt better. I made the mistake and went back for more blood work and they kept me there and scared me that I could go septic and die? I was discharged by an infectious disease **************** who told me this happens a lot, my blood work was comprimised and not bacteria and sent home. **************** only charged me $150,35. However, *** charged me another $6,836, then another $1,100. then more! Another $56.95 bill. And I was on top of it! I called and called to make sure all the bills were paid. NOW 13 MONTHS LATER DR *************************** IS BILLING ME FOR $1208.70 THROUGH A BILLING SERVICE CALLED ENVISION? *************** CALLS HIMSELF PRESERVATION PARK EMERGENCY PHYSICIANS AND THEN BILLS YOU AGAIN. My ***************************** states Dr. *************************** sent them several duplicate bills too, 4 times. And now 13 months later he still trying to get more money from me? I need this to be removed so that it doesn't efect my credit rating and *** states I have a zero balance and I do not owe any more money and I need this bill to go away and all further bills to stop! This has to be illegal! Please help me! I am currently unemployed. I can't afford this!
