      On Friday October 6th 2023, my husband and l accompanied our son ********************************* to his intake appointment at your clinic with Dr.*. The experience, to even get on the schedule, was less than pleasant and not conventional in my view. We live 1hr 15 mins away, our form of payment through care credit wasnt going thorough in your end, l had called them a couple times and they said all was well with them, my husband had to drive all the way there on Thursday to make sure you got your full required payment $500 otherwise the appointment would be canceled. Most reasonable practices would require a deposit down of $100-$150 in case of a cancellation, not full payment, this policy looked untrustworthy to me. We arrived on time for the 12:45pm youve asked to be there for. Waited about 30 mins to see Dr. ** her office was a very uncomfortable, unprofessional looking boiler room. It was so unbelievably hot, l had to step out about 1hr 20mins into the appointment because l felt like passing out. She apologized a few times, but the situation was not made better by any means. Your practice advertises itself as a functional medicine clinic amongst many other approaches. However after our disappointing experience and being told Dr. * and your main providers were functional medicine MDs, l looked them up on the *** website and they are not on there. That seems as a deceiving tactic to get people in the door at $500. The information we gathered/received at the visit was not worth that amount. We are having a very difficult financial time, it was a stretch for us to get the Care Credit to help our son. The offered treatment prices are astonishing for things that arent scientifically proven to work. The other modalities such, HBOT, lV drips etc can be found at other places closer to us for much less than what you charge. There is in my humble opinion, no need to pay those prices. Our trip there was a waste of our time, money and energy with how hot and uncomfortable we were in that tiny crowded space Dr * has. Shes not a specialist, shes a family practitioner. I am asking for a full refund to my Care Credit account $500. We work very hard for our livelihood and feel we were duped. By the way, we corrected your staff many times on how to spell our sons name. You sent a card in the mail that read *********, it shows how much you dont care for the human being. I left you a message Monday morning, waited on hold for 8 mins then the receptionist, she asked why l was calling, said you were not answering your page. Your VM says you would return the call within ***** hrs, well that time has passed and there hasn't been a return call. In the message again, l voiced my concern. Today this day, no one has made contact with me. I wrote this email on 10/9 and have called several times. Thank you, *******************************

      I received a letter in the mail stating that this organization had a data breach. My information was included in this data breach. I have emailed and left two messages with this business to contact me regarding the data breach. I have never been a patient, current or previous employee. There is no reason for this business to have any of my personal information (name, social security number, credit card number). I am trying to receive some explanation of how my name is affiliated with this organization.Thank you *********************** ************

      This medical practice is owned by ******* *********************************************** test was done about July 11, 2022, cost: $3600, and sent to Onconomics Plus, in ****** for processing. The doctor requested other local tests that were done and paid for separately. I returned to go over the results and charged a fee. The doctor discussed the local tests he requested, and tests by other doctors and wanted copies, but wouldn't request any other tests until my PSMA Scan and other outside tests were completed. During my out processing with a staff member, I mentioned the doctor had omitted the Onconomics Plus test of 23 pages but probably didn't need to cover the entire report, and could she help/ She referred me to a staff member named *****, RN.After a few calls to *****, and sending her a list of questions (See list below), she declared the report was too complicated for her to answer and suggested I pay another fee for a consult with the doctor. I explained, I felt that was unfair to charge me again. My phone calls were no longer returned.I would like either my questions answered free of charge or reduce my lab fee ($3600). Oncotrace questions:: 1. Alkylatin Agent (Page4)__Define what sensitivity means in terms of treatment. The higher the level of sensitivity, the better? 2. Inhibitor of topoisomerase (Page5)__What are the items listed horizontally on chart with low sensitivity? 3. ********************* Spindle (Page6)__Define 4. Tumor Related **** I (pg8). Bars are not high for ************ What does that signify? 5. What's significant on charts Pgs 10,11 22,23? 6. Explain tables on pg 12__17. What's the difference between high & low risk as they relate to treatment? 7. Did it show cancer cells, specifically, prostate cancer cells. Which page? How is my immune system? Understand cancer cells are often found in the blood but a strong immune sys. can keep it at bay (non-pathogenic). Correct? Add others you feel significant.

      On scheduling a new patient appointment the scheduler told me "muscle testing" by David M., M.D. was MY option. On meeting with David M., M.D., he deemed muscle testing REQUIRED and said he would not proceed with me as a patient. I spoke to the office manager. I let the office know I did not want to proceed. I had pre-paid a deposit, the receipt shows $550. David M., M.D.'s office issued to me a check as a refund that was UNSIGNED and therefore no good. I am again asking for the $550 with a GOOD, cashable check, recognizeable at any bank sent to me at the address shown. Thank you.
