      Hello,Today 09/09/24 I received from this company a bill for services performed a year ago.I did last October 2023 my mammogram and my ************* a self paid patient and benefits from the pink month ( breast cancer awareness) discount with memorial hospital south and had the two exams done for $150.I paid before hand obviously otherwise they will not see you and I have received before hand a good faith estimate from memorial hospital who assured me that I will not receive any types of surprise billing.Well they were wrong!The bill sent today for services already paid was $810 for something that cost me $150!!!! Obviously after wasting more than 30 minutes with them on the phone they realized their *********** apologies nothing This is so wrong at so many levels!They tried to be paid seven times the cost of something I had already paid! Legal actions should be done there is a name for what they are doing and it is racketing and I wont stand for it!

      This company is attempting to charge me $700+ for an ** visit, that was already paid for now twice by Florida Blue. I'm supposed to have a $200 copay for ** visits, and $50-75 for specialists.On May 3rd, 2024, I was at *****************************. I did not get the services I needed for my food poisoning and colitis, **************** was abusive to me, lying that I had nothing wrong, ridiculing me for eating pork, mocking my pain, and then calling me an a****** to her coworker. I left that hospital without any antibiotics or antiparasitic, so my condition was not treated. They hooked me up to an IV to administer morphine, something I did not need or ask for. I got things I didn't need, instead of things that I did.Fast forward, and now I am getting "surprise ****s" in August, over 3 months later, with insane charges. It isn't enough that the hospital attempted to **** Florida Blue $13,000 for this visit that resulted in nothing but abuse for me, but now the radiologist wants $1000, and the physician wants $2000. Florida Blue's supervisor informed me, that this hospital's choice to **** separately from the main claim, where I'd only owe $200 in copay, is a choice, they do not have to **** separately, they are choosing to **** separately. Florida Blue assured me, the radiologist and physician could get paid under the same claim # that the hospital **** was filed.But then this also gets to another issue, which is, how can they justify charges of $700 to me, after already being paid $900 by insurance. I'm not sure if this is the same nurse I saw at the ** that discharged me, or an error, but if it is the same nurse I saw, I'm supposed to believe that an hour or less of her time (saw her probably 7 minutes total), costs $1,600? Thats quite an hourly rate, that amounts to 3.3 million annually. I have insurance that covers me, and they are skirting it to charge absolutely insane fees. I've seen cheaper ****s in the past, uninsured, where a lot more was done.This is a scam.

      I went to **************** Dec 28, ********************************************************************** my left arm. I got et KEG and blood test and a chest Xray and *** told me I was fine. Gave me an aspirin for the muscle pain and told me to follow up with a doctor if needed. I got charged for ***** which is LEVEL 5 Extreme Case instead of Level 3 which was my case. I reached out to their customer service and they refused to look at my case and adjust the billing correct. **************** is in the process of reviewing their charges and heard back from them to help me with this Matter but Envision Refuses to get back to me.

According to my prescription provided by my primary doctor that Exam is Screening Mammogram BI ( the price co-pays too high $259.00). I have called to Envision 2 times without resolving. My account number is ****** I have not yet paid that bill since I can do it until 09/05/2024.I attach the bill from Envision Physician Services and my prescription from my doctor.       I HAD SURGERY ON 10/27/2022. ABOUT 6 MO AFTER SURGERY I RECEIVED A BILL FROM ENVISION PHYSICIAN SERVICES FOR MY ANESTHESIA DURING SURGERY, WHICH WAS TO BE EXPECTED. THE ISSUE I HAVE IS THE ANESTHESIOLOGIST THAT WAS SENT INTO MY SURGERY WAS OUT OF NETWORK WITH MY INSURANCE. I CONTACTED ENVISION AND STATED HOW THIS HAPPENED AND WHY AN ANESTHESIOLOGIST OUT OF NETWORK WITH MY INSURANCE WAS OUT OF MY CONTROL AND I DIDNT FEEL I SHOULD PAY MORE WHEN IT WAS OUT OF MY CONTROL. I APPEALED THIS. I WAS THEN TOLD THAT I WAS RESPONSIBLE AND NEEDED TO PAY. I ENDED UP PAYING $3000. THIS WAS VERY HARD FOR ME TO COME UP WITH AS I HAD OTHER BILLS DUE TO THE SURGERY AND I FEEL THAT IT WAS NOT MY RESPONSIBILITY TO MAKE SURE THAT ALL PROVIDERS WERE IN NETWORK FOR MY SURGERY.

      Effective 12/31/2020 our group became out-of-network providers with UHC. We will submit a claim to UHC for services rendered and await the explanation of benefits to determine if there is any patient responsibility. We accept the allowed amount by UHC and only bill patients for any responsibility applied towards deductible, coinsurance, or copay. We do not bill patients for any non-allowed amounts. If the claim is processed OON, we submit an appeal on the patient's behalf requesting the claim to be processed in-network. 

      At the time of service or prior to the date of service, the patient is advised at the facility of all the physicians that will be involved in the procedure. The patient is responsible to contact all those physicians and inquire if they accept or are in contract with their insurance. We apologize if the information was not given prior to the procedure. 

      Please know that per notes on 5/3/203, we spoke with insurance agent Apple reference number ************** who advised the claim processed and paid on 12/2/2022 with a patient responsibility of $3946.10 due to the patient not meeting her deductible. Per the explanation of benefits $2000 was applied towards the deductible and $1946.10 towards the coinsurance. The explanation of benefits on file does not indicate the claim was processed out-of-network, however, I will have the Insurance ******************** verify it was processed correctly.

      Our company was informed via explanation of benefits that the coverage in this case was on an "Out of Network" (***) basis, which prevents us from determining what level of in network benefit she might have in order to then determine appropriate billing. Because of the *** status from her health plan's explanation of benefits, we billed as ***. We have reached out to the patient in an effort to determine what, if any, documentation she may that will aid us in determining if her plan in accurately determined her benefits level to be ***.

      We are waiting to hear back from her.  Once we do, we plan on writing to her plan to help get this matter resolved.

      Our customer service department has been in contact with the patient, who provided us with a copy of benefits determination letter she received from her insurer.  Our company has sent a letter to the insurer regarding the insurer's error in its processing of the patient's medical claim.  While we wait to hear back from the insurer, our company will be refunding the amount we were instructed was the patient's responsibility to pay by the insurer. 

      The patient will have some ****************pay/patient responsibility that she will have to pay, however,for now we are refunding the amount paid.  However, due to the insurer's error in processing the claim as out of network instead of in network, we are unable to determine the correct amount owed. Once we are able to determine the correct amount owed, we will reach back out with an accurate bill.

       The refund to the patient has been submitted for expedited processing and will take between 5-7 business days to process before she sees the funds back on her credit card or bank card.

      I have received an email from Envision Physician Services stating that a payment of $19.56 is due. There is really no information in this email to give me any confidence in its legitimacy - no account number, date of servicenothing. Heres what the email states: Your bill from Envision Physician Services at Tampa ************************ PA is ready for payment. To view a full explanation of your ********************************** the link below:Now, Im not one who just clicks on some arbitrary link. So I went on a quest to try &verify whether or not this was truly a bill that I should be paying or some type of scam. I started by trying to locate a representative from Tampa ************************ PA. This only led me back to ************************** contact #Envision Physician Services ************** And when I called Envision, I was told I could not get any information from them until I gave them my personal information. I absolutely will not disclose my private information until I feel confident of this business' legitimacy. When I search on Envisions website : **************************************, again,I am unable to verify anything. In order to make a payment or have access to my account. The website requires an account number which I don't have. So again, I have no way of truly verifying whether or not this payment is truly mine to pay.I have to say that from reading the complaints on BBB, Im not convinced that it is. And if it is, I would appreciate being sent (mailed) an itemized bill to include an account number, date of service, service provider, & any physician information that is available.The business practices of Envision Physician Services are unacceptable. I can' t believe in this day in age, my email address is the only method to contact me, the consumer, and that the expectation of trust of legitimacy is proven only from the email I received.       I am rejecting this response because:
      The business states that a statement was sent to the address listed on the complaint. I would like to know to which address - email or physical? I can say that I have only received email statements from Envision. Again, with no legitimate information for me to know that this is a balance for which I am truly responsible. As I stated in my complaint, if this is a legitimate bill that am I to pay, I request an itemized billing statement mailed to my physical address - ****************************************************************** 
      Also, the business ****** that a text message was sent with a link to the statement. First of all, the number listed in the complaint, ************ is not a cellular number. It is a landline. Secondly, even if it was a cell number, again, I don't just click on links in messages from businesses I don't recognize or can't verify.  

      Please do as I asked & provide an itemized billing to statement to my physical address & make sure to include the information requested in my BBB complaint. And please make sure to include a remittance address as well.

      I had scheduled knee surgery dec 7, 2022. My surgeon and hospital took my BCBS of **. Insurance. I said checking in that I wanted everyone to use my insurance. After about a month it became apparent that the anesthesiologist billed my insurance but wanted way more, and was apparently not covered under BCBS?

      We have reviewed the claim for the date of service, and it shows the claim was processed by the insurance company, BCBS. Per the explanation of benefits, we received the balance of $5372.52 was applied to the coinsurance. BCBS was contacted on 6/15/2023 and we were advised the anesthesiologist was out of network, so this is why the balance is patient responsibility. Please know that I have removed the account from collections and placed on hold for review. If you went to an in-network facility our claim should be processed in-network. The collection agency will be contacted as well so they can stop all communications while we review the claim.  We would like to ask you to allow 30 - 45 business days to receive a response.       Our company does the billing for the Emergency Room physicians. The bill is for the direct treatment and/or supervision provided to you at ***************************** in the Emergency Department by a healthcare professional. These providers are independent contractors and are not employed by the hospital. Therefore, they must bill separately from the hospital.  

      We have been advised by your insurance carrier that the remaining balance was applied to your in-network deductible. We are unable to adjust any amount applied to coinsurance, copayment or deductible. If needed you can set up an interest-free monthly payment plan at billpay.envisionhealth.com.       I have already paid the medical bill for this ER visit.

      I was not informed that the physician that saw me during the visit will cost extra by a 3rd party company (Envision).

      In your response you wrote: "These providers are independent contractors and are not employed by the hospital."

      How are the patients supposed to know this? It's nothing but a common scam.

      Please remove the charges/retract the billing immediately.

Upon admission to the Emergency Department, you signed a consent form which stated that non-employed providers and/or provider groups who are independent contractors; not agents of Broward Health, will provide services to patients. By signing the consent form, you acknowledged and agreed to understanding the document.

      Based on the above, we billed for services. The remaining balance has been applied to your in-network deductible according to your health insurance policy guidelines. We are unable to negotiate any amount applied to coinsurance, copayment or deductible.

      By law, according to the Hospital Price Transparency Act, the prices need to be disclosed.
      While ************** is compliant, I can't find your prices displayed on Broward Health's site.

      I received a bill from Envision Physician Services for an Emergency Room visit over a year ago. The bill said "We submitted a claim to your insurance plan. Your insurance plan adjusted and/or paid... $0.00". As they claimed my insurance did not pay any amount, I was being charged in full. However, I checked with my insurance company, and they have no record of ever having received this claim. This means that the information in the letter is fraudulent. I went to Envision's website to try to figure out the situation, and eventually found a page where I could add my insurance information (which they did not have). There was no information about this potentially being the case. It seems like Envision is hoping that people will not double check with their insurance, or carefully look through their website, and will simply pay the bill.

      The referenced visit was identified as auto-related; as such the claim for our services was initially submitted to auto insurance. Once the denial from the auto insurance was posted, the insurance was removed. A division of our company worked to obtain the correct insurance; when they were unable to obtain the correct insurance, the billing cycle resumed.
