      On 2/23 I had a Dr. *********** with Dr. ******************************* at ********* Heart and Vascular in ***************They sent me a bill for ***** , on3/25 for a co pay of *****, which I paid on 4/1. On 4/29 they sent me the same bill saying I was past due. I got my receipt from the bank stating it was paid on 4/1. I then sent them a copy of that statement along with the copy of the statement from the bank saying it was paid on 4/1. I thought it was over.I have received another bill dated 6/3, again asking for payment of the same bill, saying again that it was past due. I tried to call them and you cannot get a person to talk to.Please help!!!! I have excellent credit and always pay my bills on time. I don't want a stain on my credit report. thank you, *************************

      On 4/11/2022 I went for a yearly visit and was charged $212.14 for the visit and $24.85 for the labs, (yearly visits are 100% covered by my insurance). I paid these amount; however, in 6/2023, I had another yearly visit and was charged another $212.14, I explained that the visit was a yearly and was told the billing would be corrected and I didn't owe anything for the 2023 visit. I asked them to correct the billing for the 4/2022 visit as well because they were billed improperly, I was told the 4/2022 claim would be adjusted; however, I am now receiving a bill for $212.14 for the 4/11/2022, when I went on the patient portal to print my receipt from 2022, I saw that the $212.14 I paid in 2022 was applied to the 2023 visit and this provider wants me to pay the $212.14 for the 2022 visit again. Fraudulent billing and business practices. I want the $236.99 that I paid in 2022 refunded and the $212.14 amount due for the 2023 visit removed as the provider refuses to bill correctly. I should not have to pay for fraudulent billing.

      This complaint is a duplicate to the complaint filed on 7/26/23 under complaint #******. The issue was forwarded to our billing department for review and it was confirmed that the amount owed of $112.25 is accurate. The billing department has spoken with the patient regarding this on multiple occasions. There is nothing further we can do for this issue.

      I am rejecting this response because: they never spoke to me about this new amount, I have attempted to contact them several times with no success. This was a yearly wellness visit and had it been billed properly, it would have been paid at 100% by my insurance and I would owe nothing. Billing for anything other than a yearly wellness visit is fraudulent and improper billing. Millennium refuses to properly bill this claim as they are looking to make more money from me. I paid them $212.14 and now they want additional money almost 2 years later? This is improper and I do not owe them anything. The response that Millennium gave is incorrect, just like the claim they filed was incorrect.

      I went to see one of the providers for an issue with my toe. I was prescribed antibiotics. My medical records indicate the I am allergic to penicillin and this includes any drugs that may have a cross reaction. I was prescribed antiobiotics and had a severe allergic reaction where both my eyelids became puffy and inflamed and my face has red marks underneath my eye. This occurred on Marc 25, 2022. I called the office and was advised to take Benadryl. One month later I still have severe red marks on my eyelids and face and still have not resolved my toe issue. I called the compliance department to discuss the matter and the office manager contacted me. I asked for a refund of my co-copay and my bill which I was refused to be reimbursed. This provider made my situation worse and have caused me harm as he did not read my medical history or my chart. I will never return to this or practice. The office manager that called me back was very unprofessional and extremely rude. The provider or office manager refuses to credit my bill. I am asking for some assistance in this matter as I would like my bill credited. I will need to seek care from a dermatologist for the issues I am having with my eyelids and face.
