My family and I moved to Gulf Breeze, FL on 7/29/2021 from Alabama. Upon attempting to enroll my children in the Santa Rosa School District, I was told they both needed current physicals done in the state of Florida. On 8/02/2021 I called my insurance company ******* (X-XXX-XXX-XXXX) and asked if they would cover school enrollment physicals done at urgent care because I there was not enough time to get them seen by a PCM prior to school starting on 8/09/2021. ******* assured me that they would pay for the school enrollment physical at an urgent care center. I took my children to Ascension Sacred Heart Urgent Care at Tiger Point and upon checking in I informed the front desk staff I brought my kids for school enrollment physicals. I indicated "School enrollment physical" on the forms I was given, and verbally explained to PA Crawford that my kids were there for "school enrollment physicals" as required by the Santa Rosa County School District, and that neither of my kids played sports, nor were they looking to try out for any sports teams. PA Crawford incorrectly coded this visit for both children as Z025, School Sports Physical. My insurance paid for the physical for my son but denied the claim for my daughter (their mistake obviously for paying for that code). However, I continue to receive bills for the incorrectly coded visit for my daughter. After 8 months of bills and phone calls to Tricare, Ascension Sacred Heart's billing department, and the Tiger Point office, I have not been able to resolve this issue.
