      This dentist replaced 3 crowns. I never received a dental plan after calling and asking for it during visits. The dentist charged me a WARRANTY FEE FOR EACH CROWN 3x289=$867. I never asked for a dental warranty and didn’t sign anything to get one. He is trying to get more money because he had me pay $7496 upfront because he was going to do 2 additional teeth but I decided to go to another Oral Surgeon because Dr S***** lied about being able to give me General Anesthesia and I found out that he didn’t have a license for that. He told me he would put me completely asleep for the 2 extractions. I am glad I decided not to go forward with him doing my 2 extractions. I never heard of a warranty charge plus he never mentioned it and again he never provided a dental treatment plan. He added on other charges that he didn’t tell me about but I will live with that. I just want $867 back because there is no evidence that I asked for this. A warranty should be a patient’s decision and like I said he never gave me any documentation to support this warranty charge.

      The patient had treatment completed (detailed on statement attached) on 11/29/22. She was pleasant before, during, and after her treatment. She told staff that she was satisfied with services. After her treatment was completed, we received a refund request through Merchant services (document attached) for a large amount of the cost of treatment of $6496. The patient did not involve our office with this at all and never requested a refund from our office, nor let the office know that she was unsatisfied with any of the services provided. The office also went above and beyond normal standards and filed and processed her out of network dental insurance for her, at her request. The office normally only files in network insurance. After her insurance made payments (as seen on statement), a refund of patient's overpayment was given of $2341.50. After patient received the refund from the office, she then questioned charges on her statement, which were completed 11/29/22. The charges were explained to her multiple times. The patient became very difficult to talk to whilst screaming and threatening staff. Communication was written from then on. The patient was also aware of the IV Sedation that she was receiving. Please see attached IV Sedation consent form.

      I am rejecting this response because I never agreed to a warranty charge and I do not have any documentation that has me agreeing to a warranty charge of $289 each for 3 crowns = $867 . I want proof of this agreement because I never even heard of a warranty charge on dental work and would never paid for anything like this.  They never gave me a credit until I filled a dispute a dispute on American Express.  I already sent you the bill for his charges I paid $8,128 witch was for 5 teeth. He also told me that he had a license for providing General Anesthesia. I found the proof on a doctors website. That is why I decided not to continue my work with him to extract 2 molars.  I was upset because they said that they didn’t know how to apply credit but after they got a dispute letter in late February. The problem was that the came up with this warranty charged and then kept giving me sedation when you go in and out of awareness. If he stopped the sedation while making a mold it would of be easier to make the mold and he charged an extra $400.  He is trying to steal my money.  Also they  never provided a treatment plan after asking for one before and after the work so I never knew what he was charging me until late February after the dispute with ****.He came up with a build up charge during my last visit. In my opinion he is a liar and is trying to extort me.  I already sent you a bill. So I never got a treatment plan or agreed to the warranty fee. He needs to provide proof that he sent me a treatment plan and I agreed to a warranty that costed $867.

      March 1st, 2022, to Aug.15th, 2022. On March 1st, 2022, I paid $ 1,868.00 for a new upper denture. After the above elapsed time frame a wearable new denture was not provided. Mant visits and grueling fittings yielded an unwearable and unacceptable denture. The Dr. also has exhibited crude movements during all the attempted fittings, and his obvious lack of sanitation protocol was disturbing, his gloved hands containing grinding debris was transferred to my mouth several times. After several months of failed fittings, the Dr. did send out my 47-year-old upper denture for a tooth repair, without charge. The repair to the old denture is all I have to show for the $1,868.00 charge, the Dr. refuses to comply with my reasonable request for a refund of $ 934.00, offered a much less amount. I believe the cost is not the sole issue, sanitation should also be considered an important issue, within any dental practice.

We made a response after his first letting explaining to return the denture and we would make a partial refunds. This letter is also attached. The Dr has fabricated the statement of signed forms for each fitting, also untrue statement that he lined and repaired my old denture twice. This is more than reasonable reviewing the visits and efforts made to please the patient and not lack of the denture delivered to him. Besides the bold lie of relining my old denture twice, when it was not relined once, only repaired with two teeth. The old denture fits perfectly, always has, a recline did not take place, period. The new denture is unwearable, more crude than the first two attempts. The offer of $ 668.00 is not reasonable, so the Dr. Is forcing me to file two separate claims with the state of Florida, one for not providing a promised and acceptable denture, one for the Dr's crude and repeated lack of heed for sanitation protocol, which will probably close his practice for pending investigation. He will find further actions far more costly than my requested $ 934.00.
