I was told I would receive a refund by manager and I have not received anything yet. I gave $1000 after initial consultation and I decided not to proceed before I received any treatment all I had was a Free consultation, As the first time that no one called or refunded me my funds I am still waiting. I left text, and called and left message and once again unprofessionalism again no one with the exception of initial correspondence with the manager Bruce I am still waiting to receive my refund to which I was told I would receive it with 5-7 days.Business response

Re: Dispute Clear Choice 1101 N Lake Destiny Suite 30 Maitland, FL 32751 321-282-6595 The transaction started on November 3, 2020. I hereby seek redress for pain, suffering and breach of agreement with falsified documents. I am disputing my disapproval of services rendered by the above entity as follows: My surgery was on hold as I needed medical clearance from my physician since bloodwork results were not available. Because I could not do the surgery at the time, I was scheduled to have two teeth removed because of intense pain. All X-rays were done, the dentist Jorge D********, made a mistake extracted a good tooth and left the affected one in place for another 3 months, causing me to suffer greater pain. The initial cost, as I understood, for the procedure was $21,000 for a plate of removable upper dentures and an implanted bottom set. Financing became available for me from the said company. Later, I learned of an additional $9000: this caused the disruption of service, and I am now stuck without any sense of direction. I have an altered document in my possession with signature and the incorrect spelling of my name, and where I should have initialed to verify acknowledgement of the additional amount, there is no mark. One office manager searched for this document and was unable to find it before the altered version showed up. I am kindly requesting your assistance with this matter, as I have reached out to the company, and no one has responded. Sincerely. ******************************Business response

The claimant was seen for medical treatment and as such, given the confidential and sensitive nature, our response is limited in detail out of our obligation to follow privacy standards. We believe we have done everything possible to resolve the claimant's issues and we do not feel there is anything further we can provide at this point. We would be more than happy to speak with the complainant at the local level should they wish to communicate further. Consumer Response /* (3000, 11, 2022/02/25) */ TO WHOM IT MAY CONCERN One Mr. Bruce O******** called and didn't leave a message...I called the number back and his secretary told me that he was in a meeting and would get back to me..for 3 days now I have been reaching out to him to no avail...I am not satisfied with their response and I have a witness to attest to my report and the falsified document that they presented to me.
