      I was approved for sovereign immunity (Charity) by Brevard Health Alliance to receive a medical procedure done at ************************************ on 6/11/24. On 6/4/24 and 6/6/24 I underwent some necessary tests required before I could get the medical procedure done. These tests included an ekg and labwork on 6/4/24 and 2 cat scans done on 6/6/24. These tests are now being billed to me by Brevard Physicians Associates. I spoke to them and they asked that I provide documentation proving that the costs of these tests was approved for payment through BHA sovereign immunity. This required documentation was faxed to them on 7/17/24 and emailed on 8/5/24. The documents included approval letters for payment. Today, I received a reply to the case they established for my dispute case number ******** stating that while they received my documents, they require that I apply for charity through them. This is completely illogical as BHA has already stated they are paying for these costs totaling $3,252.00 as they are part of the medical procedure done. I need your help because Brevard Physicians Associate is expecting me to pay monies that BHA previously agreed to cover.

      On august 11, 2022 I received a $768 for medical services provided. I made two payments one for $368 and one for $400 on august 13th, both of which posted to my bank account 8/14. On September 11th, I received a bill for the same statement stating I owed $400, I was confused as I had already paid it and contacted ************ which is the number on the statement and spoke to someone that said I was refunded $400 for a duplicate charge and owed the money. The situation was escalated to "supervisors" stating they would call me back. I never heard anything and assumed it was taken care of. On November 17th I received another statement stating I still owed the $400 so I called back again. Sat on hold to be told there was no supervisors and I would get a call back. The representative Alejandro stated he had no one higher to connect me to, there is always someone higher and refused to budge to connect me to any one. 3 months later and I never received the refund they stated I did and yet my account still says I owe them money that I don't.

      I have submitted 6 requests for a copy of the HCFA/CMS 1500 form for my daughters Date of Service of 5/20/2022. My Daughter is a minor. Her name is ************ and her Date of Birth is *********. My initial request was on *********. I then put in additional requests on ********************************************************. I was told each time that my request was escalated to a supervisor (would not let me speak with said supervisor) to fulfill my request. I was told I would get a call with an update and have never received a phone call. The company had no problem sending this document to my personal insurance company for payment but I need to have a copy of it to send to a 3rd party for victims compensation and I cannot get this company to send it to me. I have had not problems getting the same form from other companies for many years. Because this particular documents needs to be forwarded on to the Victims compensation through the court I needed it timely and Brevard Physician Associates has been unable/unwilling to assist.

      I received a debt collection letter from CMRE Financial Services Inc, in regards to Brevard Physicians Assoc, PLLC-Radiology Melbourne Florida. Account Number ********** for the amount of $28 owed as of 12/09/2021. I have no acknowledge of this account or debt. I never received any statements that I owed to pay a bill. I have no knowledge of this place.

      I received a bill from Brevard Physician Associates dated 9/9/21 for $205. I paid the bill online (receipt attached) on 9/17/21. In early November 2021, I received an explanation of benefits from my insurance company indicating that they also paid the $205 - the notice is dated 10/31/2021. I called Brevard Physician Associates in early November 2021 and requested a refund. I was told it would take about two months. This is April 2022 and I still don't have the refund. I have called numerous times and they have acknowledged I am due a refund but have yet to send it. I spoke to a supervisor as well. They all promise to call back but do not. I would like my refund ASAP. Supporting Documentation is attached. Thank you.
