            Betty Ford Hazelden did not complete a psychiatric evaluation within 72 hours of intake, which is against DCF guidelines 65D-30. However, they charted that I did. I met with with a provider with a RN in the room for 15 minutes during my stay. That isn't an eval. The provider I met with briefly also prescribed multiple medications that were not discussed with me and this is after I told them I only wanted to take my one psychotropic medication. I was in RTC LOC and their BHT staff does not do Q15s, also against DCF law. My clinician met with me for 30 minutes in 5 days and did not actually complete a bio-psych. He took the pre-assessment that admissions did and then asked me a few updated questions to create a bio-psych for charting purposes. Clinician was going to call my wife for an introduction and never did after 4 business days. They also have no co-occuring groups related to MH. Several groups are videos with a BHT sitting in the group room for observation. They have client that's are authorized PHP in RTC LOC and mix PHP authorized clients and RTC authorized clients together. Total lack of services and compliance.
