This is the first invoice I have ever received for a standard wheelchair that was delivered to me last year. My insurance company made the arrangements for me; I have never spoken to anyone at Sunbelt Medical Supply of Orlando. This invoice to me is dated September 28, 2021. Invoice XXXXXXXX is dated 09/14/2020 for $7.36. A second invoice, XXXXXXXX dated 10/14/2020 for $5.76, is included with the first invoice. The "Insurance on File" is incorrect for the date the service was received. I question why it has taken this company over a year to notify me of any amount due. I had a different insurance policy in effect for 2020. Did they even submit their claims to my insurance company of record in 2020? The phone number for Billing Questions is XXX-XXX-XXXX. Thank you in advance for any help you can give me.
