      H333**534353831313537H patient since **** Acc ******** ** ******/******, in 2020, I moved to ***** *******. While in ** I had excellent services, in ***** the quality of service were much worse. Both ******s belong to ****************************************************** Holdings. Managers treat patients with hostility and disrespect no effort to correct their own mistakes .Front desk are unprofessional, rude and insulting patients in public. doctors/managers change every 3-6 months. Doctors blame the previous one for bad work. doctors, managers change every 3-6 months and the new ones had no idea what the previous one did or promise to do I had horrible experience at the *****, ** *********** ****** with 2 crowns that fall down shortly had to be reglued. The 2 of them installed at the wrong angle/thickness front teeth #8,9 by ******************************** on 7/14/2021. Those wrong crowns caused my lower front teeth to wear down unevenly and needed restauration w/ fillings. *************** left the ******.The new dr *************************************** 7/19/2022, decided to replace the wrong crowns #8.9 and restore the lower #**,24,25,26, but he went in vacation before my app so ************************** on 11/16/22 from the other ****** in *****-****, ** *******************, took over and finally replaced the wrong crowns with new ones and fixed #**,24,25,26.I was charged two times for the same crowns #8.9 and one time for the damaged lower teeth caused by the wrong upper teeth crowns. All 3 different managers assured me it will be credited to my acc as soon the insurance payments will process-Despite my numerous emails, ph calls, in person meetings I received no credit -All 3 managers promised me each time (***********************************, *******************************, and ***********************) a meeting was not successful. I called H33343334373834343**9H corporate ********** that also promised to resolve my credit in Nov.20** I am asking legal advice that this is a case of malpractice that cause damage to my H31353**4313937303032H health and unfair double charging for the same job if not favorably resolved

      I am rejecting this response because:   H333**534353831313537H needs more time to investigate the issue--I want to make sure they understand that two offices (*******************, ** and *****, ** *********** both are independently owned and operated by ************************************ DMD HOLDINGS, PLLC were involved in this complain not only one as stated by their response

      Second I am attaching documents and pictures that show the wrong initial installed 2 front crowns $8, 9 by  ******************************** on 1/15/2021 charged $718 for both to patient and second After pictures and doc showing the removed and replaced same teeth crown by dr  *********************** on 11/16/22  and charged again double $718 each ? it is clear that dr ***************************** wrongly installed the crown at the wrong angle and wrong thickness that needed redone to correct the bite wrong angle- as shown in  dr  *********************** evaluation -that resulted in severe damage to my lower teeth that needed additional fillings provided to me at additional cost--This botched job cause me H31353**4313937303032H harm and financial loss since I had to pay second time extra $ 718x2= $1436 for same crowns plus filling repairs to lower teeth #**,24,25,26 that were healthy and undamaged before first crown install.. Managers ***********************************,
      ******************************* contacted the regional manager that promised refund for second crowns and lower teeth fillings but I never received any credit or refund
