      The dispute is regarding out-of-pocket payment done to the Chiropractic, part of the in-network **************** pretending by the same provider, that the insurance doesn't cover the service provided, but claimed 1 of my visits, at Aetna, total $170.0 (initially dated for 5/10/23, and in September 23, after my email complain to the chiropractic, changed the date to 5/30/23). Went there on 5/10/23 with horrible pain and insomnia for 3.5 weeks hopeful to recover somehow. I was asked to pay $ ***** which i rejected and told them i had insurance, after which they charged me a Copay of $50. I had a shockwave (2 **** ******* (8 minutes) therapy, the 1st visits there and had a very slight improvement, Hence, I booked 6 sessions till 6/1/23, being desperate of my pain, insomnia, and limited mobility, hoping to recover somehow, which never happened, and the therapy was neither changed from their side, since there was NO improvement of my health condition. Additionally, i was asked to pay out of pocket the total of $ 747 although the service i took all the visits, were the same one as the 1st day. ****************** told me that the insurance doesn't cover the service and asked me to pay cash (which i rejected) or credit card. Desperate and in bad health conditions i paid with credit card on 5/10/23-$50, on 5/15/23-$348.50 and on 5/30/23-$348.50. After checking with Aetna, I learned that there was NO copay for me to Chiropractic, they must claim my visits to Aetna, and I had 20 visits free of charge/year. I felt cheated, taking advantage of my situation and asking to pay out of my pocket. I wrote them email, explaining my communication with *****, asking them to claim all my visits to ***** & refund me the difference they charged me extra. They answered to my email, and added 1 more claim, of $245, but that was it. I claimed myself the total of $747, which was denied by ******** missing information from the provider. They didn't respond to many of *****'s calls/voicemails either.

      Please see the attached response to the complaint. For further information or insight you can contact me, **** Goodman, ** at ************************************** (direct email). There seems to be some misunderstanding in Ms. ********** interpretation and understanding of  her insurance plan and coverage. This has been explained in multiple emails to her as you have received. You can look back and see our explanations of coverage. I have also provided the patient notes for treatment showing improvement. Her subjective information as seen in the note is filled out by the patient on an iPad through our software and inputted into the patient note. She is stating on her own accord without doctor or staff intervention as she checks in, improvement in her condition.

      your response is not satisfactory to me, and I am still asking and waiting that all my visits to Good2Go Chiropractic, to be claimed, at my insurance company Aetna, based on the cooperation agreement you have with Aetna. 

      I have detailed my explanations on the response that i have prepared for you, as the documents attached.

      I hope that there will be a good will and cooperation from Good2Go Chiropractic to give a solution to this matter and not taking it any further.
