      My husband and I needed a few procedures and had to pay for patient expenses in advance of the treatment for what insurance didnt cover. After all dental work was done and we checked our statement and it appears we paid $700.00 to much. We are having a problem with a getting a refund. All we hear is well get back to you but no one ever does.

      I went to Monticciolo dentistry earlier in 2023 not realizing they were a chain *************** They proceeded to tell me I had to have numerous fillings drilled & refilled .What they were doing was drilling out all my fillings and refilling them even though I just spent $5,000 at another dentist 2 years before I moved the *******. Almost all of these fillings were not only unnecessary ,but when they refilled them they didn't put a numbing layer on the tooth before the fillings so now I have sensitive teeth. This was done to get me back in the again for root canals. Before my last appointment for the last filling I called to cancel the appointment and no one answered the phone late morning. I then left a message on there machine stating to cancel my appointment. I then went to another Dentist and he said i didn't need another filling , but had to redo a filling that was done in a manner to purposely trap food in between the teeth. This was done by Monticciolo even after I complained about another unethical Dentist that had done the same thing and cost me $5,000 in crowns. I was a cash customer and they made me pay half the cost of next procedure up front and pay in full the remaining amount before leaving the office. All my procedures were paid in full before leaving the office. 6 months later I was sent a bill for $77.00 and it says over 90 days. I do not owe these unethical criminals a dime. I want this bill erased and any collection action removed from my information.

      **************** balance ($77) is due to a contract adjustment that was placed after his claim was paid and closed. I have since removed it. He no longer has a balance.

      made an appointment for a follow up consultation to have 4 teeth dealt with ,was told when I made the appointment there would be no charge for the visit ,when the visit was over they charged my $313 which I paid reluctantly !!!! The staff could care less!!!I I like *********** and was going to have him do the job but NOT NOW!!! Me or my wife will NEVER go back there!!!

      ************** was seen on 3/17/22 as a new patient in our ******************.  At that time, he was given our new patient special of free exam, **Rays and a Consultation.  ************** then cam in to the ****************** for an appointment on 11/9/23.  No longer being a new patient, he was charged for the **Rays and exam, as he should have been.  He was very upset and very rude to the staff.  He has since been dismissed as a patient.

      I had work done in February 2023. And after the work was completed I received a bill for $704. I paid this bill even though I didnt feel its correct. I filed an appeal with my insurance company, and I won the case. but for some odd reason they will not return my $704. I am still working with their accounting department to resolve this. *** had several discussions with their office manager, and she agrees that the money is due back to me.

      This business keeps sending me the same statement for ***** for a dental cleaning that should have been covered by my insurance at the time. They were instructed on how to correct the dental claim so that they could send it back to my insurance company, so that it would be paid in full. I've been in contact w/my insurance company and they state that they have received additional submissions for this claim from their office but that they have yet to correct the incorrect info (my member ID). I have email threads to the business to show as proof that I've been telling them for some time now how to correct this and get this claim paid.       It started with i have gone for many years to Monticello Dentistry, in ********************* and very happy going there, When they opened their office in **************. I went there for cleanings, and then I was told all of a sudden, after getting cleanings every 6 months, and supposedly getting Dentist checks, which my dental plan was paying for. All of a sudden they said they could not make another appointment because I needed deep cleaning, I spoke with the general manager to sort this out and she made an appointment in ********************* office, to get an opinion from their dentist and hygentist, finally at that point I had ******** and paid for extra dental insurance. They told me my teeth were ok, and we never made an appointment for a deep cleaning, only a regular cleaning, but could not give me a sooner appointment. When I finally got an appointment, i had 7 fillings 2 crowns needed, and root canal. i hadn't had cavities for several years.Because I had paid for dental insurance through ******** and it changes every year, I had Argus speak with the office manager ******** to see what they paid for and what they didn't. I made 3 payments, and had the work done, and finally started to get deep cleanings. They told me that the work on 2 quads at a time, so in august 2022 I had that done, but it was either filed wrong, or incorrectly done at Argus, and apparently it was not submitted again by the Dentist. In October, they gave me the second appointment, and when i went in they were trying to charge me for august's work, even though they never resubmitted it, so i argued with them, and then they said they can only do 1 quad, because now there was not enough time. When I got home, I called Argus, and they called them and explained that they had to resubmit, then 2 weeks later, i called again, to see if they resubmitted, and spoke with *****, who told me that she has resubmitted the claim, then I got a bill from Monteccelo 4 days before Christmas, they won't treat me for my last cleaning, and Baycare, my medicare provider has switched dental plans. I just got off the phone with a supervisor from Argus, and they told me that Monteccelo dentistry keeps refiling the wrong codes. Also, I have been treated badly everytime I go into the office, since all this, not by the dentist or the assistants, but the front office staff, office manager and billing people.

      I got two crowns (tooth #5 and tooth #**) installed in my mouth on March 25th 2022 by doctor ************************ at Monticciolo Family and Sedation dentistry, Seminole locations.I was asked to prepay before the work began. I paid first time $629.00 on January, 20,2022, and then the second time again $629.00 on February, 18th, 2022.I have *************** health insurance with dental coverage of $2,000.00/year, which covers at 100% level any crowns at level negotiated with dentists. In my case the negotiated cover is $944.0/crown. My dental Insurance paid for the crowns to the clinic $1.792.0. The check for the amount was received by the clinic and cashed in October 2022. I made many requests of my itemized bill, which should show how my out of pocket money was used, but haven't got any. I also asked for a refund, because I believe I do not need to pay to the office $1,258.0, but was told by the office supervisor (or manager, I do not know her title) *************************** that I am not eligible for any refund. I was told by the front office staff that she will answer questions about my money by contacting me via phone. I called her after waiting for a few days for her to call me. She was mean to me and refused to provide any info about my out of pocket payments. She hang up on me. When I called my insurance, I was told that the insurance paid for my 2 crowns and the clinic should refund some of the $1,258.0 I paid. The clinic is in the network that insurance is working with and I became the clinic patient after my insurance suggestion.I keep requesting any calculations and explanations about my payment for the two crowns starting April of 2022, but so far all of the attempts are unsuccessful. I am very disappointed with the way the clinic is working with patients, billing and out of pocket expenses.I have hopes that you will be able to find a way that the clinic will cooperate, produce the work itemized bill, show how my out of pocket money was used and issue me a refund

                                                                   1.about not receiving an itemized bill for the service at the clinic (crowns on teeth #5 and #**, which were installed on March, 25th 2022) 

                                                                  2. Not receiving an explanation  about the use of my paid out of pocket money in the amount of $1,258.00

                                                                  3. Not getting any refund from the paid out of pocket money ($1,258.00)

                                                                  4. For the rude customer service I received from *****************************, when I was told not to call, not to complain and not to expect any refund.

      From our understanding, your insurance denied your claim as OoN, when we know we were in network. We have already tried speaking to the insurance to have the claim corrected and have been told it would be, but then it doesnt happen. After some time of this, the patient was advised to speak to her insurance as to why she now had a balance because of an unpaid claim. The insurance company has to clear this up.

      5. Claim receipt from Humana, showing that  A). ************************ and Monticciolo Family and Sedation Dentistry are in network, B) that  Humana dental insurance paid to the Monticciolo clinic for my crowns on teeth #5 and #**,

       

      Please let me know if you'd like to see any additional information about payments made by Humana to the Monticciolo clinic pertaining to the two crowns the clinic put in my mouth or any other relevant information.

      Monticciolo Family Dentistry: $3909.00, Bridge replacement: Jan. 10, 2020 Had to be removed, made improperly and didn't meet when mouth was closed, I could not chew. Feb.21, 2022 New bridge seated. Bridge didn't fit right. I went back numerous times for adjustments. Still in pain, they refused to adjust it any more with out me paying $60.00 for adjustments. They made it wrong and expected me to pay for it! Went to another Dentist to see what could be done, after filing a complaint with Monticciolo. *************************** took x-rays and found the bridge was cemented in wrong, and it needed to be replaced. # 4 has a distal open margin. The bridge need to be replaced it is not viable. My insurance company, Met Life concurred. They said I should go to Monticciolo and get my money back. I tried to do that and they hang up on me when I call. I sent letters to them with copies of the letter from both **************** and Met Life. Still they ignore me.

      On September 1st 2022 a procedure of dental veneers was performed and it was completed horribly where I stated my displeasure. The next day the veneers started to fall out of my mouth,I contacted the dentist office and had to wait days for a appointment to correct the issue. I had to go back again because they continued to fall of again . After several attempts and being ignored for a refund,the dentist while admitting I should get a refund has totally ignored me and has not given me a refund. I lost confidence in this dentistry and need my refund to go to another dentist for the service.

      Top denture badly done, & they refuse to fix or reimburse health insurance, their charging me out of pocket. I have no top denture and if course no funds. So my gums are hurting, can't eat properly and haven't had any resolution with Dental tech nor dental manager, they were paid for services not rendered.please advise
