      On 2/16, 3/9 and 3/30 of this year 2022...I got my puppy vaccine shots, rabies and heartworm test at this hospital. Since my appt on 3/30 I have requested 3 separate times for my dog's medical records showing vaccine status and heartworm results. Twice my verbal conversation and once by email. They seem to be trained not to give out this information as they know the owner will probably take them to another vet, thus losing business sales. I need this information for my dog's health records. Please help me get this information. Thank you.

      Business Response /* (1000, 5, 2022/04/25) */ Ladies and Gentlemen, We are disappointed to learn that Mr. B******* has opened a complaint against Tampa Veterinary Hospital. For close to 100 years, Tampa Vet has been providing outstanding care for the pets of Tampa Bay, and we feel Mr. B********* complaint is unwarranted. We require all records requests to be done in writing to ensure that only authorized persons are receiving this information. This was explained to Mr. B*******, and on 4/6/22, our team sent Mr. B******* the required paperwork which needed to be returned in order for us to fulfill his request. Our records indicate that on two separate occasions, we have provided Mr. B******* with the records he has requested. Once was on 4/18/22 and the other on 4/21/22. We are happy to send these records again to the email address we have on file for Mr. B******* so we can consider this matter closed. I have included a screenshot from our management software that shows the communication between our team and Mr. B******* from 4/18 and 4/21, which is prior to his filing this complaint (as well as threatening a lawsuit). We thank you for bringing this matter to our attention. It is always our goal to meet and exceed the expectations of our guests and to provide only the best care available to all of our patients. Unfortunately, Mr. B******* feels that our location is 'in the ghetto' (his actual words) and our prices are higher than he expected since we are 'on the wrong side of Kennedy' Boulevard. We wish his pet the best of health. Business Response /* (1000, 7, 2022/04/25) */ ***Document Attached*** We have once again submitted the records to Mr. B******* via email to: ************@gmail.com which is the address we have on file. The records are attached. Consumer Response /* (3000, 9, 2022/04/27) */ (The consumer indicated he/she DID NOT accept the response from the business.) This vet says they have given me my medical records but have only given me a billing receipt. They lied in their Google review response saying they provided it. Please ask this business to provide us proof, such as an email date/time stamp showing this as so. They cant because it was never sent after 3 requests. Also they violated Hippa privacy laws by using patient name and confidential details of my 3 visits on a public Google review. That's a clear violation of the law. Providing a receipt is not the same as official medical records that I can take with me to my next vet appt. A screenshot of their Hippa violation is attached for your review.
