      I received a Covid-19 Vaccination from this company on 8/28/2021. They were to report this to the state of texas and have failed to do so. I have contacted them by phone three times & email once. On my first call I was advised to email, which I did without any response. My last 2 calls I have been told they are entering the information manually. They say they are unable to tell me when they will be finished. I am not able to receive the county incentive for getting the vaccine due to their negligence of reporting. Not only is it altering data for public health information, now I am concerned of their practices all around. With all the fraud related to covid how do I know I actually received any vaccine at all. Additionally, they have not reported my husbands vaccine to the state either.

Regarding the complaint from our mutual client, we reviewed our vaccination records, and we were able to locate the client's COVID vaccine administration record. The client received the first dose of the Pfizer Vaccination in late August 2021. The client was notified to return in 21 days to receive the second dose of the Pfizer Vaccination. Due to the volume of COVID-19 vaccinations to be recorded and the complexity and diversity of electronic reporting infrastructure across the country, Emergent Testing, along with other providers, are manually entering thousands of vaccination records into the public health systems. Our Operations Manager has reached out to the client. Additionally we have sent the client an encrypted email containing a system record of the client's first COVID vaccine administration, and have offered to do the same for the client's partner.
