On September 30, **************************************************************************************************************** I saw ************************* who acted very mean to me! She asked me to move my arms and legs to see if there was any pain. There was pain in the back of my arms and legs, but she said she wasnt putting that down because I didnt mention it when I walked in. Then she yelled at me for telling her my hips hurt when I first walked in! I asked her to put it down about the back of my arms and legs hurting, but ************* also should have did a X-ray of the back of my arms and legs. Then she asked me where did I want my prescription to go: I said CVS, but Doctor ***** sent my prescription to *******, so CVS couldnt fill my prescription. I had to call ******* to cancel my prescription for it to be released for CVS to fill out which caused me not to get prescription that day! While waiting in this doctors waiting area for the nurse to call me back, this black smoke hit my knee and burned my knee, I asked for the doctor to look at my knee, but she yelled and refused to look at it! I asked the doctor to do a incident report, but she refused to do a incident report: then I called the police and they told me to speak to the manager of the business and do a incident report. The manager wasnt in on this day!Initial Complaint

I was forced to pay a 120 copay at time of service. Insurance explanation of benefits from Anthem showed that only 100 dollars was allowed and was paid by Anthem. I have called the ********* clinic twice. My information was taken but no one returned my call.I tried contacting corporate billing multiple times. No one answers the phone and the voicemail system will not accept voicemails.
