In early February, I saw a post from Forma Medical Aesthetics on their Instagram page that they were searching for a model to participate in a training for injectable fillers. I reached out to inquire, and they asked me to send photos of myself to be evaluated as a candidate for the training. I sent photos on February 6th and was selected as a great candidate for the training. My appointment was scheduled for February 20, 2023. In the meantime, I was asked to fill out waivers, a health intake/assessment and pay $1648 in advance of the treatment. This all took place without a consultation, speaking to the provider or knowing what my treatment plan was. I was assured by the office manager, ******, that I would be thrilled with the results.On Feb 20th, I went in for my treatment. The providing nurse, ********, was assisted by a visiting nurse. They asked what I wanted to achieve with the treatment, to which I responded that I would like to treat under my eyes, so I don't look tired/hollow, and was open to their professional opinion on how best to treat my concerns based on my face shape, aging, etc. I was clear that I did not want to look overfilled. They suggested to treat my temples, cheekbones, and under eyes. Trusting them as the professionals, I agreed. They completed the treatment that day and informed me about post care, that swelling would be extreme for the first 72 hours and the filler would take 4-6weeks to smooth out. I was not informed, and still do not know, exactly what product was used in my face I was told that 4.5 syringes were used to treat my face. Several days later, I reached out because the swelling, particularly in my cheekbones seemed extreme. I sent pictures, and I was assured it would be ok, and was scheduled for a follow up. At the follow up four weeks later, I still was not pleased with the results. My cheekbones were noticeably overfilled. I was told by the provider that I needed to have another treatment in my lower face that would cost $2800, and that the original treatment was "not done yet". I needed transitional work to blend the sharpness of my cheeks with my lower face. I was not aware of this prior to or informed during the first treatment. She told me my only other option was to have all the filler dissolved. I left without a solution, and hoped the filler would smooth out or reduce over time. Another four weeks later, I was still so upset with the results and hated looking at myself in the mirror, I felt like the treatment aged me by 10 years when I was hoping for the reverse. I reached out and asked to meet with the primary provider/owner **************. She was unavailable, so I went in to see my original provider, and she agreed to dissolve some of my cheekbones and a small pocket under my eye that had bubbled up. I went in for a follow up two weeks later on May 11 with Nurse ****, she apologized for my experience, agreed that I was overtreated in the cheekbones, and said she would have never treated me in that way due to my specific anatomy and the shape of my cheeks. She offered a different type of treatment complimentary should I choose to do so. I told her I would consider, and at this point I was nervous to do any more procedures.Three weeks later, large pockets of filler appeared under my eyes. On Mon May 29, I reached out to the office to ask to be seen and sent pictures. They told me it was likely allergies, and they could see my in a few days. On Tuesday May 30, I went to another office unaffiliated with Forma for a second opinion, they confirmed it was filler under my eyes that had migrated and "festooned" due to improper superficial placement. They recommended I reach out to the original provider to have it dissolved. I reached back out to Forma and asked that they see me that day. They squeezed me in with my original provider ********, who was doubtful it was filler, told me it was probably allergies, they won't know until they treat it with the dissolving agent, and that I should take antihistamines (Allegra and Pepcid) as well. I told her I wanted to proceed with dissolving. She treated the pockets under my eyes with *******, as well as dissolved more in my cheeks. Within 30 minutes of treatment the pockets were gone, and my eyes returned to normal, without taking antihistamines. I sent pictures to the office for confirmation that it was indeed filler. Throughout this experience, I was made to feel it was my fault by ******** and the office manager ******. I asked the office manager if there was anything they could do to help rectify the poor experience I had, as it had now been three months of dealing with this, causing me stress and also having dissolved most of the filler, I had nothing to show for the money I had spent. I told her I would be happy with offering of other treatments that did not involve filler, or some money back. I was not inclined to accept the offer that **** had originally provided, as it involved injections under the eyes, due to my experience, I was fearful to have another procedure that involved injections under the eyes. ****** told me she'd have to consult with **** and ****************, the proprietors, and asked for follow-up pictures. I sent pictures the day of the Hylenex treatment confirming it was dissolved, and additional pictures as requested and never heard back. I had been scheduled for a follow-up appointment 4 weeks later, and unfortunately had to cancel due to a sudden death in my immediate family. I am now hoping to take steps to reach a conclusion. Overall, the experience from start to finish was poor. From the beginning I was not given a customized treatment plan as they advertise is part of their service, in advance of signing consents or paying for the service. I was made to feel like this was my fault ****** and ********, and in the end was not followed up with about a resolution. Furthermore, I continue to have issues with improperly placed filler migrating under my eyes. To this day I occasionally have to manually move the filler around to smooth it out. My undereye skin seems to be permanently damaged and sagged due to the treatment. I would like to receive some type of refund.
