Reference Information - Wellstar Account *******, MyLabBill.com access code WR-******* I am a self-pay patient. I visited ******************* (Rheumatologist) on 5-23-22 and received an office consult and blood work. I was billed for the office visit and paid in full for these services. Total costs for the office visit were $306.40 which I paid out of pocket as a self-pay patient. There were no issues with this part of the Wellstar services.******************** ordered 3 laboratory tests on 5-23-22 as part of my office visit. When the blood tests were ordered ******************** provided me with a self pay estimate and order form that listed the lab tests to be done and the price that I was to pay. I was asked to bring the order form to the Wellstar lab to have the blood work completed. The tests marked on the form were as follows: *** w/ diff $15, CMP $20, Urinalysis $10. I took the order form to the Wellstar lab as requested to have the blood work performed. I gave a copy to them and kept a copy for myself.I have 2 outstanding issues with my Wellstar bill. Both issues relate to the charges for the lab work done on 5-23-22:1. The bill that I have received shows incorrect, elevated pricing for the 3 aforementioned tests that were disclosed and authorized. The bill shows *** w diff $39.70, CMP $83.50, Urinalysis $16.20. My bill for these 3 approved services should be $45 and not $139.40. I would like the bill adjusted.2. The 2nd, larger issue, is that I am being charged for a number of blood tests that were not disclosed to me, nor approved by me. All other charges shown on the bill are for tests that were not disclosed and not authorized. I would like the charges for the undisclosed and unauthorized tests to be removed.I am being asked to pay $986.03 for blood work that I was told would be $45. I will upload the self-pay order and the invoice both for reference.In August I filed a billing dispute with Wellstar and a complaint and heard nothing back.
