    1st Grievance Complaint: Refusal to release all my Medical Records including, Clinical Notes for all visits for medical attention for dental work received from Stone Mountain Dental My Request for Release of my Dental Medical Records started back on October 28th and I was told that I could come up to the office, complete the necessary document and they would e-mail me all of my Dental Records plus imaging that was taken because they did take pictures of my mouth several times with all the visits back and forth due to problem with the dental work done on my Mouth, but this was not true. I was told by the Dental Aide ****** that the Dentist (Dr. ********* had to review the information before releasing it to me. The Aides on that day even made-up excuses stating that the printer was out of paper and/or they could not e-mail from the front desk computer and so forth. I also emailed Stone Mountain Dental at ******************************** several times with my request; 11/3 and 11/25 via Email, called on 11/1 and went by the office on 11/2 2nd Grievance Complaint: Dental negligence delayed and poor treatment that has led to further injury or suffering (tonsillitis/bacterial infection) due Multiple Dental visit in 1 Months time (10/7, 10/14,10/17, 10/19 and 10/24/2022). *The ******************* Care Physician prescribed me: Amoxicillin-CLAV on 11/9/2022 to take for 10 days, and to treat my Tonsillitis infection. Back again, 12/3/2022 to ******************* care ************* needing another round of Antibiotics. Infection not completely gone. Doctor prescribed: Cephalexin 500 MG, Take 1 Capsule every 12 hours for 7 days . It wasn't until my visit on Wednesday, October the 19th, that Doctor ******** provided me with a prescription for 800 MG of Ibuprofen to help me with my pain. Dr. ******** also provided me a prescription for Chlorhexidine, I am unable to drink on the right side of my Mouth and at times have to use a Straw.
