I have attached a letter that I sent to Functional Medicine of ***** that explains my situation. After sending that letter, I discovered that with the No Suprise Act, by law *** should have informed me that Quest Diagnostics is an out-of-network lab. Even without considering the No Suprise Act, I signed an *** form (with *** letterhead, not Quest Diagnostics) that stated the estimated costs of 3 of the 12 labsthe actual price is nowhere near the estimate. ***********************, Co-owner (with her husband) of ***, has contacted me. I have recorded phone conversations with ***** where she explains that *** had previously had a contract with Quest Diagnostics, which gave *** patients better pricing on labs. She went on to say that her business was being audited by the *** and that a resolution to stop to auditing was to disconnect the contract with Quest and have Quest bill patients instead. She stated that it all happened so quickly in an attempt to stop the auditing and keep their businesses afloat. In a separate recorded phone call to ******** in the *** billing department, I was told that the phlebotomist made a mistake in having me sign the *** form and not telling me of the changes that were made since my last visit regarding the separation of the contracted lab Instead of owning the human mistakes made in her facility, and by her employees, *********************** keeps pushing me to figure out the situation directly with Quest Diagnostics, attempting to wash her hands clean of this mess. My desired resolution reflects my paying the full cash pay price of these labs, which I didn't intend to pay nearly as much, assuming insurance would cover a lot of it! I believe *** should be responsible for the payment beyond that. I have a few recorded phone calls with *********************** and with ******** at the *** billing department. I am unable to upload them to this form but can send them separately if needed.Thank you for your help!
