this is a bill on my credit report for a hospital contact them to pay of balance of 1829 for school for my daughter hospital bill back when o was laid if for covid and fooughef agreed to pay the balance on the report they agreed as well sent me a bill for over $3000 i never agreed to and the laid and stated once payment was made it can be removed from my credit report multiple times i denied what stated accept to pay the balance on my report ive all rude not will to work with me in a balance from 2020 during a time when everyone was struggling and laid off i want was discussed to take place anything else is fraudulentBusiness response

I'm not entirely sure what ************** is saying here, but we sent ************** the debt verification documentation on 9/23/23. We spoke to her on 6/6/24 and she agreed to a monthly payment plan. On 6/7/24, the collection tradelines for these accounts were deleted as promised.Initial Complaint

This medical collection is being reported illegally. Under the ***** law ***************** Portability and Accountability Act of 1996) to protect my privacy and medical records from third parties. I do not recall permitting EM STRATEGIES to release my medical information to a third party. I am aware that there is a debt from EM STRATEGIES, but I lack sufficient information about the validity of this debt. Therefore, I want to ensure that any reporting to credit bureaus is accurate and compliant with relevant laws. - Provide a copy of my signed authorization allowing the release of my medical information to your agency. - Confirm compliance with ***** regulations regarding the release of protected health information. - Cease any ongoing credit reporting activities related to this debt until the validation process is complete.If the requested information cannot be provided this account needs to be removed from my credit report immediately.Business response

To address ****************** HIPAA concern,Merchants' Credit Guide Company is a contracted Business Associate of EM Strategies, therefore, authorized to handle Personal Health Information (PHI)to the extent necessary to secure payment on behalf of our client, the covered entity. We have provided the debt verification documentation to ************** on two separate occasions to the address presented on this complaint: 10/27/22 and 2/28/23. ************** received medical treatment from our client, EM Strategies, when she needed their services. She has not paid for those services. We tried to contact her so we could work with her to come to some resolution. We never received a response. We reported the accounts to the credit bureaus. We have updated ****************** accounts as disputed.Initial Complaint

After non-payment, ******** ****************** referred the accounts to ** for collection. Merchants' Credit Guide Company is a third-party debt collector and a contracted business associate of **************************. We will request the account statements and the signed Authorizations and Agreements from ************************** and send them to ************. Upon receipt, we would encourage ************ to call us with any questions she may have. We remain open to work with ************ and come up with a manageable payment arrangement.Initial Complaint

I recently checked my credit report and noticed this is now listed as a collection although I thought this was resolved as I have received no further communication from merchants. This debt does not belong to me. I asked for documentation that the debt belongs to me including the signature and copy of ID presented at the time of service and I did not receive this. I believe the provider billed the incorrect party. I request this be removed from my credit report. Thanks!Business response

We have never been able to get a response from **************** since July of 2020, when we first received the account from our client. We sent letters and called the cell phone provided here, leaving several messages. We have the account statement, the signed financial agreement, and copy of the photo ID, her Driver's License. We will mail the information to her at the address provided here.Customer response

There is no contract. We sent an itemized statement of account to ****************** yesterday. She received medical treatment from the creditor. They billed her insurance. This is the balance after insurance. She is responsible for this balance. Upon receipt of the statement, we encourage her to call us if she has any questions.Customer response

We sent ****************** copies of the account information on 3/7/24. We believe this information will help her remember the treatment she received. If she still believes this is ID theft, as we stated in our letters on 12/7/22 and 2/16/24, we will need a copy of the signed ************************ ID theft affidavit, a copy of the police report about the incident, and a copy of a state issued photo ID card. He received treatment in the emergency room by a physician from Hernando County ************************** working out of ************** - ***********. The emergency room physician group sent ****************** a bill for the services provided. After non-payment of the bill, the account was sent to our company for collection. I am writing to address a discrepancy regarding a medical bill that has been reported to the credit bureau in the amount of *****. Despite several attempts to dispute this matter with both your office and the credit bureau, it appears that an error persists in the reporting. The bill in question has already been settled by my health insurance provider. Please see EOB enclosed. Given the circumstances, I kindly request the immediate removal of the erroneous entry on my credit report. It is imperative to rectify this issue promptly as it is adversely impacting my credit standing. This discrepancy, originating from Midwest Orthopedics billing process, does not accurately reflect my financial obligations.As a Nurse Practitioner operating a business, my ability to access credit is essential for its operations and growth. Therefore, the presence of this incorrect information on my credit report is significantly hindering my professional endeavors.I trust this matter will be addressed with the urgency and attention it requires. The Explanation of Benefits (***) **************** included with her complaint was for a different date of service. The date of service on the account placed for collection with our company was 11/10/21. The *** refers to the treatment received on 11/24/21. We are sending **************** an itemized statement of the 11/10/21 Date of Service so she can look for the *** from her Allied-Aetna insurance. It appears as though most of the charges from 11/10/21 were applied to ******************** deductible, thereby making the balance her responsibility. But she should confirm that with her insurance company.Customer response

I am rejecting this response because:
This bill was indeed paid the summer of 23 after I sent a copy of a MRI to show medical necessity. I did not have a deductible owed as I had broke my leg in March of 2023 and satisfied the deductible at that time. All bills with Rush were covered 100 percent except for this last one because the insurance company was awaiting radiographic evidence of joint space narrowing to show the injections were medically necessary: I supplied a MRI results to the insurance company during the month of June 2022 and they send payment out. Please get with **** and go through all the charges for the 3 injections and you will see it was satisfies.

**************** is talking about treatment/services in 2023. I clearly said the account placed with us for collection was for services rendered on 11/10/2021. We mailed a copy of the statement from our client to **************** yesterday. 2/22/2024. She should call her insurance company with that information and find out what happened that claim. ************ was picked up by an ambulance from the Darien-Woodridge Fire Protection District and taken to the hospital on 3/8/18. This account is for the ambulance charges incurred. There is no contract. She received the services when she needed them, and she is responsible for payment. The claim was billed to her ******************** and all the charges were applied to her deductible, thereby making her responsible for the balance. This debt is beyond the statute of limitations, so we cannot and will not *** her for payment. If she makes a partial payment, the statute of limitation would restart, but we will still not *** her to enforce collection. We are happy to work with ************ to come up with a manageable payment arrangement when she can.Initial Complaint

********************** received medical treatment at **************************. After the hospital billed **********************, and he didn't pay the balance due, they placed the account for collection with our company. He has never asked us for a copy of the contract. We have requested a copy of the itemized bill and a copy of the Agreements and Authorizations signed at the time of service. Upon receipt, we will mail this information to *********************** It is our hope that he will callus with any further questions he may have. We will be happy to work with ********************** to come up with a manageable plan to get this account resolved and removed from his credit reports.
