IN HOME CARE for my caregiver/homemaker to help around the house on 4/14/2025 **** at number ************ called SED i was terminated and waiting on Meridian Health to send them updated info or call them so can keep going with my caregiver/homemaker no one sent them the info they need this is wrong now no help dew to this this is wrong i get people telling me cant send paper work to company i was given at all i know you can and can call out to them i like you to call the number i given for **** and get paper work to them to keep me going.Business Response

On March 3, 2025 *** assigned me a transportation company to take to my medical clinic. I had a 12:15 p.m. appointment. The transportation company was ****************** as at 10:40 a.m. the owner of the transportation company contacted my phone stated the driver was there for over 20 minutes as I refused to go out. I told the owner I been at home all morning as no driver ever showed up, called when they landed or came to my door. The transportation company didn't ask me for directions or verify my address or ask what nearby my home. So A&I transportation cancelled my ride without sending another car or sending back to ***. Accordingly with my insurance guidelines all transportation matters needs to go through *** in which I called for an emergency ride. A *** *** by the name of ***** plus a supervisor to which I never spoke with or obtain his/her name canceled my ride without researching that I had a ride canceled or verify anything with myself nor my doctors office there solution was I need to book another day with the doctors office as they were not going to assist me with this emergency. *** failed to realize there are penalties for rebooking last minute as also no call no shows with medical facilities . Since *** failed to properly assign me another transportation company. I had to pay $38.96 in which I provided receipt below for ************************** to review. *** has often did this in the past as their answer was I need to contact my insurance for "gas mileage reimbursement" in which this is not the case this is reimbursement for a ride in which MTM failed to provide for myself. I also sent a copy to A&I transportation if someone from *** would like to get a hold of A&I transportation and work something out they are welcome to do so. The complainant will need to submit a complaint directly with their health plan. Their health plan will then work with *** to investigate and resolve the concern. My Health plan is Meridian Health of Illinois part of Illinois ********* The main reason I did not add my health plan is MTM here is responsible with all aspects of transportation for the state of Illinois and with Meridian Health plan. *** is under contract with Meridian as Meridian will just respond MTM is handles this as it's their responsibility. MTM book all medical rides, they can reschedule, as they are the go to people if a transportation company cancels on their end. The people under ******** reach out to MTM the only time we go through our health insurance (Meridian) is when we file a grievance (complaint) with MTM. If you ask anyone in Meridian they will tell you same thing that *** is responsible with this matter. If the BBB look at the other complaints other members from other states having the same issue so this not the first time this happen. Now as Mentioned if MTM would like to connect with A&I transportation since they were the company *** assisgned to me as A&I transportation one who canceled in the first place. Meridian keeps harassing me with useless questionaire that has nothing to do with my insurance policy and threatened to kick me off cause i refuse to keep answering the exact same questions every month or two. I have been with meridian for over six years ever since ***** made it law for people to get insurance. They have never once called me or bothered me until last year when i had to start getting meds for high blood pressure and allergies. They even herassed me into downloading an app that they wanted me to get to keep harassing me with their useless questions that has nothing to do with my insurance or policy. How many times i exercise, what i do for a diet or any other personal activities is none of their business. The questions they keep asking literally has absolutely nothing to do with anything they are required to know. They never bothered me before but have started harassing me since last year and now they threaten to kick me off the insurance cause i will not answer questions that they have no reason to be asking. What i do for my health is only for me and my doctor to discuss. The only thing they need to know is if everything I use my insurance for is covered by them. They have herassed me over 20 times for the stupid questions. By law they are required to pay me for every time they have herassed me plus more for the stress they have caused.Initial Complaint

My disabled son aged out of my group employer health insurance and was told he now had to be on a ********************* health plan because he had no other health plan. The State of ************ unit confirmed that he was no longer on my **** before they would let me choose a **************** plan for him.Meridian ***************** has denied all his claims because they are said to be "submitted to primary insurance first." There is no primary. If there were, he would not and could not be on Meridian. I have tried to submit documentation from ****, my employer, and the State to Meridian, but all I get is the customer service running around or just telling lies. Deny claims and encouraging fraud by asking people to file claims with insurance they are not on must be investigated.Business Response

I have Meridian health for my mltss long term care, and they move my caregiver service to other company and i didn't hear who it was from my case manager at all so i called here she just gave me the company phone number and I called company and *** they talked to Meridian health my case manager and explained they need it to do to my long term mltss preauth and then preauth send to them the referral to company they moved me to but not doing that cant start me until this is done been week/ month since no caregiver either dew to this neeed to get preauth from longterm care insurance and then preauth eather fax or email the referral after preuath dose there thing to company who meridian health moved me toInitial Complaint

I am filing a complaint against Meridian Health of Illinois as a provider. I received a letter on 4/30/24 demanding reimbursement of overpaid claims in the amount of $843.40. My provider contract is less than 5 months old and I have been paid at 3 different rates for the exact same services. Theyre stating they paid more than my contracted rate however I have never received a fee schedule from Meridian. I have no idea what my contracted rate is. I am also continuing to receive payments processed at the highest of the 3 rates which I will inevitably have to pay back. My employees receive a percent of the payment from insurance and it is impossible to pay them accurately for their work. I will end up underpaying them which is in violation of the contract I have with my employees or I will over pay them and then have to take back the money. I have been a provider in other insurance networks for over 10 years and I have never experienced such incompetence. My Meridian contract does not expire until 2026 and I would like to find a way to discontinue my work with them. This is not worth the stress, time, or money of my business.Initial Complaint

I had an appointment today at 9:00 am, the transportation app said the driver would be here at 8:17 am. Despite calling several times with no response, the driver decided to show up at 9:05 am. How is this acceptable? I have been keeping a laundry list of all the times transportation errors since 2021 where someone said they would pick me up and eventually did at the same time as my appointment. It's a smack in the face when you all told me to take an uber next time, how about your transportation comes before time to take people to their healthcare provider. Today I was supposed to have an MRI of my brain and spine, but your driver never showed up. How is that acceptable? I have heard countless of other people complain about the same thing. Do we need to get together collectively and go to the media to tell them about what's going on? I cannot wait until August so that I can switch my medical provider because you have no empathy for people with dire situations who need to get to the doctor.Initial Complaint

I received mail to select the provider in March 2024 and the new insurance card will be sent. I had called member services in March as well to confirm my selection and was said yes. I received the new card in April which has a different doctor *********** is not the first time this has happened. If we're asked to select a provider then we do so to get the doctor we want. Meridian always puts some random doctor as PCP on my card and it's such a hassle to get it changed. I waited 2 months and now again will have to wait until they change my PCP. Sometimes twice they have made mistake and assigned me wrong PCP in the past. This is ridiculous.
